- This is a combined synopsis/solicitation for Commercial Items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. The Department of Veteran Affairs Medical Center, San Antonio intends to sole source purchase order to Medtronics ENT.The requirement will be issued by Department of Veteran Affairs, Medical Center, Valley Costal Bend. This combined synopsis/solicitation and notice of Intent to Sole Source serves as a request for quote (RFQ) for supplies and will be issued as Full and Open. The solicitation document, incorporated provision and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-66. The associated North American Industrial Classification System (NAICS) for this requirement is 339112 Only electronic submission will be accepted, offeror must include, CCR Registration, Tax ID, and reference Solicitation Number in Subject line of e-mail correspondence, no question will be answered. Medtronic, Baush & Lomb and GE Healthcare are manufactures of ENT surgical sets, each are among the highest quality. Medtronic's is the preferred, as the surgeons at the Health Care Center are trained on Medtronic ENT sets have familiarity using set by Medtronic for procedures being performed.
