- This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (Hubzone, 8(a), Small Business, Small Disadvantaged Business, Veteran Owned Small Business or Service Disabled Veteran Owned Small Business) relative to NAICS 624190 (Size Standard $7 million). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this sources sought synopsis are not considered responses to any potential solicitation announcement. The Department of Veterans Affairs VHA Network 15 Contracting Office is seeking information concerning possible sources to provide Employee Assistance Program (EAP) for diagnostic and referral services located at the Popular Bluff VA Medical Center 1500 N. Westwood Blvd, Popular Bluff, MO 63901. The VA intends to award a contract to a qualified institution to provide these services for the period of time of 06/01/2013 thru 05/31/2014 base year, with four follow-on option years. A Request for Quotes will be posted on FebBizOpps after the VA concludes its market research to the availability of possible sources. The Statement of Work below provides a summary of services required. All interested firms shall provide the following information: company name, address, point of contact, phone number, email address, capability statement, business size and socio-economic classification. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. Poplar Bluff VA Medical Center STATEMENT OF WORK Contract for Employee Assistance Program 1.The contractor shall provide an Employee Assistance Program (EAP) for diagnostic and referral services to an estimated 575 eligible employees (expected number of employees using the service is 5 - 10, past history has been 2 - 3 employees per year using the service) of the Poplar Bluff Veterans Affairs Medical Center hereinafter referred to as PBVAMC in accordance with all terms and conditions identified in the contract requirements, clauses and addendums. The PBVAMC consists of a primary medical center located at 1500 N. Westwood Blvd., Poplar Bluff, Missouri 63901. There are five (5) Community Based Outpatient Clinics (CBOC), two Clinics in Pocohantas, Arkansas and Salem, Missouri. Employee Assessment, Counseling and Referral services, as described below, will be provided at the contractor's place of business by licensed professionals. Authorized providers of services must be within a fifty (50) mile traveling distance from each location listed below. Services will be for one (1) base year and four (4) option years. Problem assessment, initial counseling toward identifying and assessing the complexity of personal problems which may affect job performance. Follow-up counseling will be limited to a maximum of three (3) visits per identified problem, per person, per year. Family counseling is considered to be dependents and family members as eligible if they meet the dependency and familial relationships that are recognized by the Internal Revenue Service (IRS). Written or verbal task orders will be issued by the Contracting Officer Technical Representative (COTR) or designee as referral for services. 4.Services provided to Poplar Bluff Veterans Affairs Medical Center (hereinafter referred to as PBVAMC). 5.Communication in the hospital for employees such as brochures, posters, orientation meetings, benefit fairs, quarterly newsletters including promotional in-services addressing applicable EAP topics. The contractor shall provide internet website access available for all PBVAMC employees to provide benefit information. PBVAMC shall have the option to request changes to the website content as needed. The contractor shall develop and conduct separate initial EAP awareness training to all shifts (day, evening, night) as needed for each of the following: Supervisors/Managers, American Federation of Government Employees (AFGE) Union officials, Equal Employment Opportunity (EEO) staff, and Non-supervisory employees. The contractor shall provide a representative on-site, once per quarter year, available to answer general employee questions and discuss relevant topical information about the EAP. Additional training may be required during the contract period specifically addressing mandatory drug testing, or other similar mandated policies, for both employees and management staff. 6.Initial assessment and referral for: 1)Psychological counseling 2)Relationship counseling 3)Marital and family counseling 4)Alcohol and substance abuse counseling 5)Stress, job or personal 7.The contractor shall provide general services from 8:00 AM to 4:30 PM, Monday through Friday, excluding Federal Holidays. This does not preclude services after the stated hours (e.g., emergency crisis intervention) and there shall be no additional charge for after-hour services. The contractor shall provide accessibility such as local locations as well as outlying areas, including evening hours as well as day time hours for appointments, and have twenty-four hour/day emergency telephone number for immediate professional help including weekends and holidays. The contractor shall forward medical emergency calls to 911 when applicable. The contractor shall respond to non-emergency requests within 3 workdays. In emergency situations, the contractor shall assure an appointment is made the same day. Routine appointments shall be established within 24 hours of initial contact and occur within 3-5 days or at the client's earliest convenience. 8.The contractor's staff shall have a master's level Ph.D. credentials and previous experience in a wide range of personal problems such as alcohol and drug counseling, marital and family related issues, and experience/expertise in resolving workplace problems and delivery employee assistance and related managed care services. 9.The contractor shall furnish proof of liability insurance in an amount not less than $1,000,000 per occurrence. 10.The contractor shall provide quarterly reports on usage trend for management that may help identify possible problem areas with PBVAMC. 11.The fee per hour will be paid as a fee per referral and will be paid by Government Visa Purchase Card on the month following receipt of services. 12.No information shall be provided to any person/persons, agency/agencies, in or out of the Federal Government which might identify an employee or a member of his/her family without a voluntarily signed release of information by the employee or his/her family member (or legal guardian if a minor) specifying to whom, and for what reason, the information is to be released. The contractor shall establish a confidential record-keeping system which is in compliance with federal standards, and with requirements relating to the confidential nature of patients' records in accordance with Section 333, Public Law 91-616 (as amended by Public Law 92-282), Section 406 of Public Law 92-255, and federal regulations of the Special Action Office for Drug Abuse Prevention (21 CFR 1401). 13.The contractor shall submit monthly billings to the address indicated. Billings shall notate the number of hours being invoiced. No names or social security numbers shall be listed. 14.This is a Healthcare Competitive Procurement. Attachment 6 of the Veterans Heath Administration Standard Operation Procedure (SOP) 160-10-01 Procurement Process. 15.General Requirements: a.Federal Holidays: The Federal Government observes the following as national holidays. New Years Day, Martin Luther King's Jr. Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, and any other day specifically declared by the President of the United States. b.Payment: The contractor agrees to accept payment by the Federal Government Purchase Card (Visa), Reference XX, Payment by Third Party. c.Evidence of Insurance Coverage d.Before commencing work under this contract, the contractor shall furnish to the Contracting Officer, a certificate of insurance indicating the coverage outlined and containing en endorsement to the effect that cancellation, or any material change in the policies which adversely affect the interests of the Federal Government in such insurance shall not be effective unless a 30-day advance written notice of cancellation of change is furnished to the Contracting Officer (CO). e.In lieu of insurance coverage required, the contractor may furnish evidence of financial responsibility in the form of a qualified self-insurance program an irrevocable letter of credit, or a letter in which the contractor agrees to accept the financial responsibility and further states that he/she is financially able to meet all claims up to the amount specified. The furnishing of such evidence of insurance coverage of financial responsibility may not be waived. 16. The contractor shall ensure all personnel performing under this contract are fully qualified and have the requisite state licensure, training and experience to provide the necessary services. The contractor shall provide, upon request of the Contracting Officer CO and COTR, documentation of qualifications, training and experience, of personnel performing under this contract. Failure to ensure personnel meet the necessary qualifications may be cause for termination of the contract. Qualifications of contractor personnel shall be subject to review by the PBVAMC Chief of Staff and approved by the PBVAMC Director. The contractor agrees to assign referrals to appropriate, qualified personnel and will notify the CO or COTR in writing prior to proposed substitutions. 17.The contractor shall provide performance survey forms to all employees at the conclusion of counseling services provided. Surveys shall be provided to the COTR in analysis format on a semi-annual basis. The contractor shall provide statistical reports on a quarterly basis. The report shall include demographic information, as well as types of service provided. The reports' content shall be at the discretion of Human Resources Management Service (HRMS) and is subject to change. The contractor shall issue verification of attendance on all PBVAMC employees referred by other than self or if employee requests leave to attend counseling. The contractor shall maintain complete, individual case files for all employees referred for assistance, consistent with industry standards. Case files shall be maintained in accordance with the confidentiality requirements of PL 93-282 and the implementing Federal Regulations, section 2.11(n) of 42 CFR Part 2 and PL 93-579 (Privacy Act). 18.If requested by the Government, contractor agrees that all personnel performing under this contract shall provide a financial disclosure statement detailing any financial interest in any community treatment facility or other resources to which employees might be referred as a result of counseling under EAP.
