- SF Form 1449 Solicitation Equipment Inspection, Testing, and Certification Equipment Update Report Repair Work Report PM Checklist Memphis PM Index Eastover PM Index Windy Hill PM Index Tampa PM Index Jacksonville PM Index Montgomery PM Index Huntsville PM Index Section M THIS IS A COMBINED SYNOPOSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6 STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL ITEMS-AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE.THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; PROPOSALS ARE BEING REQUESTED ANDA WRITTEN SOLICITATION WILL NOT BE ISSUED. THIS SOLICITATION IS BEING ISSUED USING SIMPLIFIED ACQUSTITION PROCEDURES. This solicitation is a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-66. Submit written proposals on RFP number W912HP-13-R-0021. The associated North American Industrial Classification System (NAICS) code for this procurement is 561210 with a small business size standard of $35,500,000 employees. This requirement is a 100% Small Business Set Aside. The Government intends to award a Firm-Fixed-Price (FFP). The full text of a FAR Provision or clause may be accessed electronically at http://acqusition.gov/comp/far/index.html Delivery: Delivery terms shall be FOB Destination Period of Performance: The Period of Performance shall be a base year, and four, one (1) year option periods. Instructions, Conditions & Notices to Offeror Evaluation Criteria: Award will be made to the best overall proposal that is determined to be the most beneficial to the Government. This "Best Value" determination will be made using the trade-off process described in FAR Part 15. This process permits tradeoffs between price and technical ("non-price") factors and allows the Government to accept other than the lowest priced offer. The award decision will be based on a comparative analysis and an integrated assessment of proposals against all source selection criteria in the solicitation. Evaluation methodology for this procurement consists of three (3) factors: Corporate Experience, Past Performance, and Management and Technical Approach.
