Description

Contractor shall provide all labor, materials, transportation and supervision necessary to provide Yoga Therapy Services at the CAPT James A. Lovell, Federal Health Care Center in North Chicago, IL. This project is subject to the Service Contract Act. This solicitation is 100% set aside for small business. The applicable NAICS code is 611620 and the size standard is $13.0 million. Award will be based on the best value to the Government. Evaluation factors include TECHNICAL CAPABILITY, price, and past performance/similar experience. The solicitation package will be available on or about June 3, 2013. The date for receipt of quotations is scheduled for June 10, 2013 at 15:00 c.s.t. All relevant solicitation documents may be accessed for download on the date of issue at the following website: http://www.fbo.gov. Any amendments to the solicitation will be posted solely to this web site. It is the responsibility of the offeror to verify the number of amendments issued. Seeking Award of Firm Fixed Contract with a Base Period of 1 Year and Four 1-Year options to extend REQUIREMENTS AND DELIVERABLES: The Contractor shall be required to provide the following deliverables: 1. Two, one-hour sessions, four times per week for fifty-two weeks (Totaling 416 session hours) of Hatha-based Yoga Therapy. The program must incorporate 5 practices: Disciplines, Breathing, Posture, Meditation and Gratitude. Sessions will be provided at: 3001 North Green Bay Road North Chicago, IL 60064-3048. Maximum of 15 participants allowed per session Must provide yoga therapy services for a beginner's level participant. Each session will be 60 minutes in duration. 2. Contractor (or designated therapist) must be a certified yoga therapist graduating from 200 hour yoga training program, should have at least 1000 hours and three (3) years of teaching experience since graduating from the 200 hour yoga training program. 3. Contractor (or designated therapist) must have experience working with Veterans patients who may have the following diagnoses: Chronic Pain, Traumatic Brain Injury, Anxiety, Insomnia, Depression, Drug Addictions, or Anger issues. 4. Contractor (or designated therapist) must have experience in working with patients who have Post Traumatic Stress Disorder (PTSD) and must be able to provide proof of such experience. PRIOR TO DELIVERY OF SERVICE: Session dates will be finalized no later than 30 days after contract award date along with locations of services. Session dates may be subject to change.
