Description
This is a sources sought notice only; it is not a request for quotations or offers. This sources sought notification is for the Department of Veterans Affairs Network Contracting Office 9 for Veteran Medical Centers (VAMC) located in the Tennessee Valley Healthcare System (TVHS) comprised of the Alvin C. York Campus in Murfreesboro, TN and the Nashville Campus in Nashville, TN; the Huntington VAMC located at 1540 Spring Valley Drive in Huntington, WV and the Memphis VAMC located at 1030 Jefferson Avenue in Memphis, TN to determine the availability of Small Business (preferably Service-Disabled Veteran-Owned and/or Veteran-Owned) Entities within the area, relative to NAICS code 562112, with a size standard of $35.5M, and possessing the skills and capabilities necessary to provide Hazardous Waste Collection services. The Contractor shall provide all labor, material, equipment, transportation and supervision necessary to provide Sharps Services for all VISN 9 facilities. Contractor shall furnish sharps containers compliant with Infectious Medical Waste Program and all sharps shall be properly disposed of in accordance with all local state and federal regulations. Containers shall be picked up weekly at the Medical Centers, contents emptied and then cleaned and processed for reuse. Contractor shall furnish all labor, equipment, and provide installation of reusable container housing in all areas designated by the Contracting Officer Representative. The contractor is required to change out reusable sharps containers at least once a week from the Medical Center. The contractor will provide monthly tracking report to identify weight generation rates. Report will entail cart weight; gross container weight; net container weight; breakdown of containers used by size; total containers used; and total pounds of plastic diverted from landfill. Contractor shall ensure that the handling and final disposal of sharps waste is in compliance with the Department of Transportation (DOT), Environmental Protection Agency (EPA), Tennessee Department of Environment and Conservation (TDEC), Food and Drug Administration (FDA), and applicable Federal, State and Municipal governmental regulations and codes. This shall include: I. Transport Vehicles: A.Vehicles shall be equipped with fully enclosed leak-resistant cargo body, maintained in a sanitary condition and secured when left unattended. B.Vehicles shall bear the transporter's name, State permit and the words "Medical Waste" or "Regulated Medical Waste" on the sides of the body. C.Waste shall not be subject to mechanical stress. D.Regulated medical waste shall not be combined with other waste. II. Marking Requirements: A. Transporter shall provide waste-resistant labeling on all containers of regulated waste with the following information: 1.Name of Transporter or immediate handler. 2.State permit or identification number of the transporter. III. Intermediate Handlers: A. No intermediate handlers or subcontracting of transportation shall be allowed. IV. Manifest Requirements: A.Transporter shall sign, date and furnish a copy of the manifest prior to any Medical regulated waste leaving the pickup sites of "Sharp Waste" generator. B.Transporter shall certify that the manifest data matches the load to be removed. C.Transporter shall deliver the waste to an authorized facility and sign the manifest to verify that "Sharp Waste" has been delivered. D.Transporter shall maintain a copy of the manifest for their records. E.Transporter shall return all remaining copies of the manifest to the generator, assuring final disposal. This request for information (RFI) is for planning purposes only and shall not be considered as an invitation for bid, request for quotation, request for proposal, or as an obligation on the part of the Government to acquire any products or services. Your response to this RFI will be treated as information only. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of contractor submission or responses to this announcement or the Government use of such information. No funds have been authorized, appropriated, or received for this effort. The information provided may be used by the Department of Veterans Affairs, Network Contracting Office located in Murfreesboro, Tennessee as a requirement for Hazardous Waste Collection services for VAMC TVHS, VAMC Huntington, WV and VAMC Memphis, TN in developing its acquisition strategy and statement of work/statement of objectives and performance specifications. Interested parties are responsible for adequately marking proprietary or competition sensitive information contained in their response. The Government does not intend to award a contract on the basis of this RFI or to otherwise pay for the information submitted in response to this RFI. No phone calls will be accepted. Interested parties should respond to the Contract Specialist, Carlos Suggs via e-mail at Carlos.Suggs@va.gov, not later than (NLT) June 6, 2013 at 2:00 PM Central Standard Time (CST). All interested parties should include the following information: 1.A company profile, including name of business, DUNS number, and other pertinent information. 2.A description of capabilities. 3.The business size and business socioeconomic category. Please note, all SDVOSB and VOSB must be verified by the office of Small Disadvantaged Business Utilization Center for Veterans Enterprise (CVE) prior to receiving any award.
