Description
This is sources sought announcement only. The purpose of this notice is to conduct market research and obtain information from qualified potential offerors. No formal solicitation document exists at this time. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. Durham VAMC (DVAMC), Durham, North Carolina has a recurring need for Mortuary Services for remains of deceased patients who were receiving hospital, nursing home or domiciliary care at the VA expense. Average number of burial is 12 per year. The applicable NAICS Code is 812210 with a size standard of $7.0 Million. THIS IS NOT THE COMPLETE DESCRIPTION LIST: The Contractor shall complete mortuary services for the remains of deceased patients who have expired while an inpatient in the Durham VA Medical Center or in a VA contracted nursing home. See list below. These veterans have no next of kin or other person claiming the remains and there are not available sufficient resources to cover burial and funeral cost. The Contractor shall pick up remains within eight hours of initial call. Pickup locations include the following counties: Craven County Durham County Optional Facilities include: Greene County Johnston County Wake County A.MORTUARY SERVICES SHALL CONSIST OF THE FOLLOWING: Preparation of the body shall consist of properly embalming, dressing, cosmetizing, and casketing the deceased. Appropriate clothing for male veterans would be a clean sports coat, dress shirt, pants, undergarments, and socks. Female veterans should be clothed in a dress, blouse, skirt or pants, and undergarments. Veteran's eyeglasses shall be worn if available. Casket at a minimum shall be cloth covered made from corrugated fiberboard, pressed wood, or softwood, finished interiors and exterior handles. Cardboard or press paper or similar materials are not acceptable. Material must be of sufficient strength to support the weight of an adult body. All casketed remains shall be draped with the United States Flag furnished by the VA. Remains shall be inspected by the Chief of Health Administration Service--- or their designee prior to leaving the contract funeral home for interment. In the event the services or merchandise are found to not be in compliance with said agreement the contractor shall be requested to correct the situation after which another inspection will be conducted. Upon completion of inspection the contractor shall transport the veteran to the nearest National Cemetery with available grave space for interment. Contractor will complete all required notifications, permits, and certificates in accordance with North Carolina state and county laws of death rules and regulations. Contractor will provide proof of license for the state of North Carolina for the establishment and funeral directors prior to commencing any work under this contract. Contractor shall not be paid for any work performed or costs incurred that reasonably could have been avoided. Contractor shall be available to provide service 24 hours a day, 7 days a week, 365 days a year. Responses to this sources sought notice should include the following: name and address of company; business size (large or small); identify whether or not firm falls into any of the following socioeconomic categories - 8(a), HUBZone, Service Disabled Veteran-Owned; brief capability statement of qualifications and any other pertinent information, which demonstrates ability to meet the above requirement.
