Department of Veterans AffairsMemorandum Special Notice This is not a request for proposal or a request for quote. There is no solicitation documentation available Under F.A.R Part 6.302-1. This is a notice of intent; The Southeast Louisiana Veterans Health Care System intends to negotiate a sole source firm fixed price contract with Provel Inc. located at 2082 Bettas Rd. PO Box 302 Cle Elum, WA, 98922. Provel Inc. which is the sole source provider of services for Provel T7 Automated Thermoformer and Provel C7 Carver equipment. The statutory authority for the sole source procurement is 41 U.S.C.2 53 (b) (c). Oral communications are not acceptable in response to this notice. Interested parties may identify their interest and capability to respond to this requirement no later than June 10, 2013, @ 10:00 PM CST. Responses must be in writing and must provide clear and concise documentation, indicating an offerors ability to meet the required services. Responses will be emailed to winston.graber@va.gov Emails are not to be in the excess of 5MB. Prospective offerors shall complete electronic annual representations and certification at http://orca.bpn.gov, shall be registered in the Central Contractor Registration (CCR) database at http://www.ccr.gov prior to award of a contract. Statement of Work (SOW) 1.0 Background The Southeast Louisiana Veterans Healthcare System (SLVHCS) is interested in purchasing a Provel T7 Automated Thermoformer and Provel C7 Carver located at Baton Rouge South 7850Anselmo Lane, Baton Rouge, LA 70810. 2.0 Objectives The goal of this equipment is to fabricate artificial limbs for Veterans. 3.0 Task The system should have the following capabilities: o Carve shapes in-house "Fully automated 3 axis CNC servo carving machine designed to quickly and accurately cut models for socks, AFO's and TLSO's as well as flexible covers. "A BK socket model takes about four minutes to carve with an accuracy of.010" (.25 min). "Thermoformer is a fully automated prosthetic socket forming machine designed to use industry standard socket preforms for fast, consistent results. 4.0 Delivery Provel shall provide confirmation of shipment and delivery date no less than two (2) business days prior to delivery to Southeast Louisiana Veterans Healthcare System, Louisiana at no additional cost to the government. 5.0 Government-Furnished Property None 6.0 Security Considerations None 7.0 Travel Travel will not be required. 8.0 Special Material Requirements None 9.0 Other Unique Requirements and Considerations None 10.0 Place of Performance Baton Rouge South 7850 Anselmo Lane Baton Rouge, LA 70810 11.0 Period of Performance None
