Description
STATEMENT OF WORK Conversion of Armoires in BLDG 2105 Scope of Work: The contractor shall provide all labor, material, equipment, transportation and supervision to perform the following work: Conversion of Armoires into TV Stands on the first, second, third and fourth floors of Building 2105 at the Naval Station Mayport FL. 1. GENERAL: The contractor shall utilize cutting tools, stain, appropriate glue and attachment devices and screws to cut and convert 260 TV armoires into 260 TV stands. 2. PROJECT INFORMATION: a. Project Description: Armoires are located in Building 2105 in the Unaccompanied Housing complex. This is a 4 floor building with 130 suites. Rooms will be occupied. The building has an elevator. 1. The contractor shall provide professional design services to ensure the removing of the top, trimming the sides, replacing of the top to ensure the TV stand has a professional finish and is appealing. 2.The contractor shall be responsible for measuring the armoires ensure correct pieces are cut, trimmed, reattached and properly stained. Armoires are currently 36 Wx75 Hx24 D. They need to be converted to 36 Wx38 Hx24 D. VENDOR WILL NEED TO DETERMINE THE CONVERTED HEIGHT. c. Project Schedule: 1. The project shall start no later than 1 month after the award of the contract. The project shall be completed no later than 15 business days from commencement of work. Contractor shall provide onsite supervision throughout entire project. 2. The contractor shall be responsible for moving the armoires to a safe working area to complete the project. The contractor shall ensure trash disposal bins are set up and removed upon completion of project. The contractor is responsible for all subsequent damages incurred while performing their service. All trash will be removed from NAVSTA MAYPORT. The contractor will be responsible for all clean up of any items falling from the dumpster during transport. 3. MEASURING AND CONVERSION: It shall be the complete responsibility of the contractor to obtain accurate dimensions at the job site and to examine all conditions as they relate to this conversion. Skilled carpenters shall perform the conversion. All work shall be accomplished in a neat and professional manner following established trade practices. They shall leave the area clean and free of all debris resulting from this conversion. The conversion crew shall notify the owner, before commencing the conversion, of the proposed schedule for the owners approval. Additionally, crew shall obtain a signed "letter of satisfaction" from the owner or his designated agent, prior to final departure from the job site. The conversion crew will bring the tools necessary to complete the job and will clean up at the end of each day and before leaving each room. 4. GUARANTEE: Conversion warranties shall apply. The guarantee shall be guaranteed to be free from all defects in materials or workmanship for a period of not less than one year from the date of delivery. All products found to be defective within the warranty period shall be repaired or replaced, with the cost of replacement, including shipping charges, shall be borne by the contractor. 5. SAFETY/SECURITY/PARKING All workers coming to site shall pass a security check conducted by Navsta Mayport Security. Prior to coming to base, the contractor must contact Unaccompanied Housing to give a list of names of personnel who will be working the site at least two weeks in advance. Workers will be accompanied by Unaccompanied Housing personnel at all times. Workers shall be allowed to park in designated areas under direction of the Unaccompanied Housing Manager. 6.0 Labor: a. All laborers must speak and understand English. b. Onsite Project Manager must speak, write and understand English fluently. c. Employees must be approved for access. All contracting personnel shall check in through the base Security Department and shall wear an approved contractor badge while onboard NAVSTA Mayport. d. Contractor is authorized to use MWR restaurant facilities. e. All personnel shall properly observe morning and evening colors. 7.0 Hours of Operation: Work shall be full time during the government s regular business hours, which are Monday through Friday between the hours of 8:00 a.m. to 4:30 p.m., except federal holidays. The contractor is not required to provide services on federal holidays. The federal holidays are Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day and Labor Day. 8.0 Transportation and Lodging Accommodations: a. The Contractor is responsible for making all travel arrangement to and from NAVSTA Mayport. 9.0 Security Clearance: Work under this order is unclassified. The contractor shall comply with all applicable security regulations and procedures during the performance of this work order. 10.0 Delivery of Goods: a. Delivery shall be FOB Destination to Jacksonville, FL. Deliver to: Bachelor Housing Naval Station Mayport Bldg 2105 Baltimore St. Jacksonville, FL 32228 For delivery arrangements/directions call: Willie Atkins at (904) 270-3264 or David Hixon at (904) 270-5575 11.0 Place of Performance: The place of performance will primarily be at Unaccompanied Housing Building 2105 Baltimore St., NAVSTA Mayport, FL 32228. 12.0 Contract Performance: The period of performance is as follows: Delivery dates to be coordinated with UH Manager. Debris will not be disposed of on the NAVSTA Mayport installation however at a location selected and funded by the contractor. 13.0 Warranty: The contractor shall extend to the Government the full coverage of any standard commercial warranty normally offered in a similar commercial sale, provided such warranty is available at no additional cost to the Government. Acceptance of the standard commercial warranty does not waive the Government's rights under the "Inspection" clause nor does it limit the Government's rights with regard to the other terms and conditions of this contract. In the event of a conflict, the terms and conditions of the contract shall take precedence over the standard commercial warranty. The standard commercial warranty period shall begin upon final acceptance of the applicable material and/or services listed in the Schedule. The contractor shall provide a copy of its standard commercial warranty (if applicable) with its offer. The warranty covers a period of _________. (Offer or is to insert number)."
