TITLE: Maintenance and Service Agreement Support of Teleform and Rightfax Equipment VHA National Telehealth Training Center, Boston, MA This solicitation is issued in accordance with FAR 6.203, 15.101-2, and 19.502-2. This solicitation is issued as a Request For Quotation (RFQ). The RFQ number for this requirement is VA101-13-Q-0155. This RFQ is set aside 100% for Small Business (SB) concerns. Service Disabled Veteran Owned Small Business (SDVOSB) concerns and Veteran Owned Small Business (VOSB) concerns will be considered in the competition as well (per VAAR 819.7004 and 819.7006). The applicable NAICS code is 423430 and the SBA size standard 100 employees. REQUIREMENT DESCRIPTION: See the attached solicitation document VA101-13-Q-0155 and VA101-13-Q-0155 Attachment 1: Performance Work Statement (PWS) for applicable terms and conditions and requirement details. This solicitation is for maintenance and upgrade services for the existing Teleform and Rightfax equipment used by the Veteran Affairs Training Center for patient and clinician education. Placement of a single Firm Fixed Price (FFP) purchase order is anticipated. A period of performance of total three (3) years (base plus two options) is anticipated. QUOTE CONTENTS AND FORMAT: Interested vendors must provide an offer in accordance with the terms of Section E (page 13) of the solicitation. QUOTE S SUBMISSION: All quotes shall be submitted electronically via e-mail. Accepted attachments are Microsoft Word (.doc &.docx), Microsoft Excel (.xls &.xlsx), Adobe (.pdf), and Image files (.jpeg,.tiff, &.bmp only). Note that the total size of all attachments per e-mail should not exceed 6MB to prevent possibility of loss of information. Multiple e-mails are acceptable. Additional instructions are provided in the solicitation document VA101-13-Q-0155. All offers shall be received no later than 3:00 PM EST on 11 June 2013. It is the offeror's responsibility to ensure that all content is included in the offer and that the offer is received by the Government.
