Description
Sources Sought Guldmann Lift Maintenance Purpose - The Department of Veterans Affairs, Veterans Integrated Service Network (VISN) 16 intends to solicit and award a contract for maintenance of Guldmann Patient Lifts for a base year and four option years. Type of Contract - The Government intends to award a single Firm, Fixed-price Indefinite Delivery, Indefinite Quantity contract. Contract Requirements - Contractor shall perform required annual weight and function tests of up to 1812 Guldmann ceiling lift systems throughout the South Central VA Health Care Network (VISN 16) in accordance with ISO 10535 and manufacturer requirements. VA Medical Centers under this contract are located in -Alexandria, LA; Biloxi, MS; Fayetteville, AR; Houston, TX; Jackson, MS; Muskogee, OK; New Orleans, LA; and Shreveport, LA. Contractor will provide all parts, equipment, material, and labor including travel cost to perform the tests and correct any deficiencies identified by the tests. The contractor shall provide all labor, supervision and all other resources required to deliver the requirements stated herein, except as may otherwise be specified in this statement of work. Performance Period: Lifts should be tested in the timeframe noted below in Table 2. Facility COTR will work to assure timely access to lifts. "include the following information: obusiness name, DUNS number and business address opoint of contact to include e-mail address and telephone number osubcontractor's business names and addresses oa representation of business size and Socioeconomic Category (HUBZone, 8(a), Service Disabled Veteran, Small Business, Large Business, etc.) All Responses are to be sent via regular mail to the Point of Contact and received no later than 1:00 pm local time on Monday, June 24, 2013. No submissions will be accepted after this date and time. All responses shall be in the English Language. Information provided will not be returned. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of contractor submission of responses to this announcement or the Government use of such information. This request does not constitute a solicitation or grant authority to enter into negotiations to award a contract. No funds have been authorized, appropriated, or received for this effort. Additional Information: Responses to this notice are limited to twenty-five (25) pages in length. Pages are to be 8.5 by 11 inches in size and will be numbered. Interested parties may include a single cover sheet and a Table of Contents which will not be included in the page count. Any submission containing more than 25 pages will be truncated after 25 pages and the remaining material will not be considered by the Government when trying to decide if the company is capable of performance.
