- The Portland OR VA Medical Center (PVAMC) is seeking the following professional services: Project Scope: This contract is required because Portland Path&Lab (PLMS) needs reference lab services for a genetic test that will determine a Veteran's risk for hereditary breast and ovarian cancer (HBOC). This is a proprietary/patented test developed and performed at Myriad Genetics Laboratories. This test will aid identifying the risk for hereditary breast cancer. Veteran samples are assigned a unique bar code for robotic specimen tracking. DNA is extracted and purified from peripheral blood samples or buccal mouthwash samples, and then submitted for molecular testing. Firms offering a response to this notice should ensure that their registration in the System for Award Management (SAM) database https://www.sam.gov/portal/public/SAM/ is current and should ensure that the applicable NAICS code (621511) is included in the SAM profile prior to submission of quotations. Method of Award - Cascade Procedure 1. Any award resulting from this solicitation shall be made using the following cascade order of precedence: 1.1. In accordance with FAR Subpart 19.14 and PL 109-461, award under this solicitation shall be made on a competitive basis first to an eligible Service Disabled Veteran Owned small business (SDVOSB) concern, provided that there is adequate competition among such firms. 1.2. If there is inadequate competition for award to a SDVOSB concern, award shall be made competitively to an eligible Veteran Owned Small Business (VOSB) concern. 1.3. If there is inadequate competition for award to a VOSB concern, award shall be made competitively to an 8a or HubZone small business concern. 1.4. If there is inadequate competition for award to an 8a or HubZone concern, award shall be made competitively to a small business concern IAW FAR 19.5. 1.5. If there is inadequate competition for award to a small business concern, award shall be made on the basis of full and open competition considering all responsive offers submitted by responsible business concerns. The anticipated Base period of this contract shall be from date of award, through 09/30/2013. There shall be 4 possible option years to exercise, depending on the needs of the VA.
