Description
The Pacific Regional Contracting Office (PRCO) Korea at Brian Allgood Army Community Hospital (BAACH)/121st Combat Support Hospital intends to award a firm-fixed price, personal service contract for the Special Needs Coordinator, Social Worker services at 51st Medical Group (MDG), Osan Air Base, Korea. This requirement is for a period of performance from 1 September 2013 through 31 August 2018 (Base & 4 Option Periods). Required qualification of the Special Needs Coordinator, Social Worker includes: (1) Have a Masters of Social Work (MSW) degree from an accredited graduate school of social work. (2) At a minimum, the provider must maintain current certification in either the American Heart Association Basic Life Support (BLS) (Course C) or the American Red Cross CPR/BLS Course as required by AFI 44-102, Community Health Management, Tables 1.1 through 1.6. The MTF will provide BLS refresher courses. (3) A minimum of two years full-time experience in care coordination, discharge planning or case management in a medical setting within the last four years. (4) Must have knowledge and skills to effectively apply SNC functions: a) Assessment: Identification of patients that need special medical and education needs/management; comprehensive collection of patient information and medical status; and continued evaluation of an established plan of care; b) Planning: Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care; c) Facilitation: Care coordination and communication among all involved parties; d) Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care coordination is received. The proposed acquisition will be unrestricted. This solicitation package will be available on both the Army Single Face to Industry (ASFI) Acquisition Business website (https://acquisition.army.mil/asfi/) and the Federal Business Opportunities website (http://www.fedbizopps.gov/)on or about 18 June 2013. No paper copies of the solicitation will be issued and no solicitation mailing list will be compiled. The Contracting Officer is Ms. Jean K.
