- Attachment 4 - Clauses and Provisions Attachment 3 - Past Performance Questionnaire Attachment 2 - Price Worksheet Attachment 1 - Statement of Work A. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This procurement is being issued in accordance with the procedures in FAR Part 12 and 13.5. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. B. The solicitation number is HSCG23-13-R-MMZ215 and is being issued as a Request for Proposals (RFP) under NAICS Code 334118 (size standard: 1,000 or fewer employees). The solicitation document, and the incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-65. The United States Coast Guard (USCG) intends to procure automated merchant mariner credential printers (passport-style) and on-site maintenance and repair. As set forth in more detail in the provision at FAR 52.212-2, Evaluation - Commercial Items (Jan 1999), the USCG intends to conduct an evaluation of the proposals submitted to determine the offeror that represents the best value to the Government. The Government provides an addendum to the provision at FAR 52.212-2. The contract will have a base period of 120 days from date of award (for delivery and installation), followed by four (4) option periods of one (1) year each (for maintenance and repair). The Government reserves the right to hold negotiations. All qualified and responsible sources may submit proposals which shall be considered by the agency. C. Offerors shall provide prices for the items on the attached Price Worksheet (attachment 2). D. The delivery location of the printers and the primary place of performance for the on-site maintenance and repair is the USCG National Maritime Center. The address is: 100 Forbes Drive Martinsburg, WV 25404 Within 120 days of contract award, the requirements outlined in sections 5.1.1.1 - 5.1.1.7 of the SOW shall be completed. E. The provision at FAR 52.212-1, Instructions to the Offerors - Commercial Items applies to this acquisition. A completed copy of the provision at FAR 52.212.3, Offeror Representations and Certifications - Commercial Items, must be submitted with each proposal. The clauses (including any addendums) at 52.212-4, Contract Terms and Conditions-Commercial Items and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, apply to this acquisition. Additional contract requirement(s) or terms and conditions (such as contract financing arrangements or warranty requirements) determined by the contracting officer to be necessary for this acquisition are consistent with customary commercial practices. F. The following attachments are included and are hereby incorporated into this solicitation: (1) SOW, (2) Price Worksheet, (3) Past Performance Questionnaire, and (4) FAR and Department of Homeland Security Acquisition Regulation (HSAR) Clauses and Provisions.
