Description

Sources Sought: This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 623220, Size Standard $14 Million. The Network Contracting Office (VISN 2) is seeking a potential qualified community-based contractor to provide Safe Haven/Low Demand housing and supportive services for chronically homeless Veterans with severe mental illness and/or substance use disorders in the Canandaigua, New York region. Proposed facility must be within 15 miles of the Canandaigua VA Medical Center, which is located at 400 Fort Hill Avenue, Canandaigua, NY 14424. Key Requirements: "Acceptance of treatment cannot be a condition of admission or continued stay. "Abstinence from alcohol or drugs will not be a condition of admission or continued stay "Must be able to serve both male and female Veterans, and provide segregate, safe accommodations to support their security and well-being. "Room and board to include a bed and other furnishings, such as a dresser, storage (to include at least one lockable drawer), and personal linens (towels, bed sheets, blankets, and pillow.) "Residents must be provided with basic personal hygiene products. "Meals to include at least three (3) nutritionally adequate meals a day, 7 days a week and availability of nutritious snacks "Laundry facilities and supplies for residents to do their own laundry or have laundry done for no additional fee. "Must be able to provide structured group activities as appropriate, such as social recreational/peer support groups, Alcoholics Anonymous, Narcotics Anonymous, linkage to vocational services/counseling and physical activities. "Supportive case management, including guidance on self-care skills and adaptive coping skills for independent living in collaboration with VA program and community resources. "On-site staffing 24 hours a day, 7 days awake, providing awake supervision of residents. "Ability to accommodate immediate admissions 24-hours per day, 7 days per week with VA referral. "Ability to transport Veterans to scheduled meetings and medical appointments related to the recovery plan of the Veteran when public transportation is not available or is not appropriate for a Veteran. Offerors meeting the capabilities necessary to provide the above stated services are invited to submit responses to this sources sought notice and must include the following information: (1) General information about your company including name, address, contact person, phone number, fax number, email address(s), DUNS number and company website if available. (3) Location of proposed facility. (4) Does your company have experience providing Safe Haven/Low Demand model of service delivery?
