- Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. This solicitation number is NICHD-13-042 and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2005-66. The North American Industry Classification (NAICS) Code is 541511 with a size standard of $25M. However, this solicitation is set aside for small business. This acquisition is being conducted using Simplified Acquisition Procedures in accordance with FAR Part 13. Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD) intend to procure on a sole source basis with TJP Software Services - 1574 Autumn View Lane - Howell, MI 48843-7688 to provide computer support services/consultation to meet the research, clinical and administrative goals of the Perinatology Research Branch (PRB). The PRB is located at Hutzel Women's Hospital/Detroit Medical Center, 3990 John R. Street - Detroit, MI 48201. The computer support services are needed on a part-time basis for a total of 600 hours with the period of performance of one full calendar year. The support services are required on-site (office space will not be provided by the PRB) at the PRB location. The services needed are "hands on" and cannot be done over the phone or wait for personnel to arrive on-site. See attached Statement of Work (S.O.w.) and Network Application Attachments for further information. Services to include but not limited to: (1) CLINICAL DATABASE: Develop, document, maintain and update a computerized unified patient medical record data management systems to track patients and data, etc. (2) GRAPHICS REQUIREMENTS: Provide high quality diagnostic images from ultrasound, MRI, CT and other modalities contained in the clinical data base, etc (3) ADMINISTRATIVE DATABASE: Develop, document, maintain and update procedures and guidelines (with input from Project Manager) for an administrative database, etc and (4) OTHER SERVICES: Provide technical support guidance and problem resolution and installation for hardware and software for computer systems and network application. Design, document and implement custom applications and hardware configurations for PRB user needs as assessed. Train PRB and Contract personnel in use of software and statistical programs relevant to PRB goals, etc. TJP Software has been providing part time computer services/consulting in support of the PRB computer network, hardware and databases. The PRB has its own network, servers, storage, tape backups and other infrastructure. The PRB has highly technical requirements to accommodate the storage of ultrasound images, interface with the hospitals electronic medical records, coordination of clinical and specimen databases, computer support for lab equipment and general desktop support for all the PRB computers. Since we are off NIH campus, all of these services are procured from vendors in a close proximity to the PRB. Given the complexity of the existing network and infrastructure and the vendor's already established technical relationships with Wayne State University and Detroit Medical Center (where the PRB is housed), it would require significant time, effort and expense before another vendor could be in a position to provide these services and resulting in a duplication of work/effort. This situation could lead to delay of services and could cause PRBs work stoppage/delay on certain programs or research. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under authority of the 41 U.S.C. 253(c) (1), FAR 6.302. This notice of intent is not a request for competitive quotations however, all responses received, within 15 days from the date of publication of this synopsis will be considered by the Government. The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-1 Instructions to Offerors - Commercial Items; FAR Clause 52.212-2, Evaluation - Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items; and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items - Deviation for Simplified Acquisitions applies to this acquisition. The offeror must include their Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. Interested vendors capable of furnishing the government with the service specified in this synopsis must submit two (2) copies of their quotation/proposal to the ADDRESS LISTED BELOW if responding via postal mail or other mailing services. Offers must also be accompanied by descriptive literature and/or other information that demonstrates that the offer meets all of the foregoing requirements. Quotations will be due fifteen (15) calendar days from the publication date of this synopsis or by July 1, 2013 by 10:00 a.m. EST. The quotation must reference "Solicitation number" NICHD-13-042. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted in writing to Eunice Kennedy Shriver National Institute of Child Health and Human Development, 6100 Executive Boulevard, Room 5Z00, Bethesda, MD 20852.
