- Brand-name or Equal Justification Statement of Work - Cartagenia Bench Platform Software (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format specified in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is NHLBI-CSB(HG)-2013-185-MKL, and the solicitation is being issued as a Request for Proposal (RFP). Offerors shall submit their proposals no later than 5:00 p.m. Eastern Standard Time (EST) on Friday, June 21, 2013. (iii) The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-66 (April 1, 2013). (iv) This procurement is being conducted on a competitive basis, as a total small business set-aside. The associated North American Industry Classification System (NAICS) code for this requirement is 511210, Software Publishers, and the corresponding Small Business Size Standard is $25.0 million. This acquisition is being conducted in accordance with the authority set forth in FAR Part 13-Simplified Acquisition Procedures. (v) Contract Line Item Number 0001: The Contractor shall provide a one-year on-site license to the Bench Next Generation Sequencing (NGS) module for five (5) users. The Contractor shall provide the necessary staff to facilitate development requests from the National Human Genome Research Institute (NHGRI), Undiagnosed Diseases Program (UDP). Contract Line Item Number 0002: The Contractor shall provide a one-year license of the Bench CNV module, and shall include access for five (5) users and up to 1600 tests/patients per annum. Contract Line Item Number 0003: Annual maintenance and support. (vi) Please refer to the Statement of Work, provided as an attachment to this solicitation, for specific details of this requirement, to include the minimum functional specifications of the software. (vii) The place of delivery is: National Human Genome Research Institute (NHGRI), Undiagnosed Diseases Program (UDP), 9 Memorial Drive, Building 9, Bethesda, Maryland 20892. (viii) The provision at FAR Clause 52.212-1, Instructions to Offerors- Commercial Items, applies to this acquisition and is hereby incorporated by reference. All Federal Acquisition Regulation (FAR) clauses may be viewed at http://acquisition.gov/comp/far/index.html. (ix) Proposals submitted in response to this Request for Proposal (RFP) will be evaluated according to the minimum functional specifications of the software, as specified in the Statement of Work, attached to this solicitation. The award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the technical acceptability standards for non-cost factors. (x) In accordance with FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items, offerors must complete annual representations and certifications on-line at https://www.sam.gov/portal/public/SAM/. (xi) FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items applies to this acquisition. (xii) FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. (xiii) This acquisition is a brand-name or equal, for Cartagenia Bench Platform Software, in accordance with FAR Subpart 16.505(a)(4), Items Peculiar to One Manufacturer (brand-name). Please refer to the brand-name or equal justification, provided as an attachment to this solicitation. (xiv) The Defense Priorities and Allocations System (DPAS) and assigned rating does not apply to this acquisition. (xv) Offerors shall submit their proposals no later than 5:00 p.m. Eastern Standard Time (EST) on Friday, June 21, 2013. The proposal must reference the RFP Number: NHLBI-CSB(HG)-2013-185- MKL. All responsible offerors may submit a proposal, which if timely received, shall be considered by the agency. Responses must be submitted electronically to Ms. Melissa K.
