- 1. REQUEST FOR INFORMATION (RFI): THIS IS NOT A SOLICITATION; no award will be made for this effort. This request for information is for informational purposes only; this is not a Request for Proposal (RFP). All interested parties are encouraged to respond to this RFI. The Government will not pay for information and materials received in response to this RFI and is in no way obligated by the information received. Companies who desire to participate in this market survey are encouraged to submit an information paper which supports their company's claim that it presently has the technology, qualifications, and capabilities to satisfy the deliverables described below. The information paper should discuss the company's ability to develop and or provide the requested annual maintenance service support for the Keller Army Community Hospital (KACH), at West Point, NY. 2. SCOPE. Background: The mission of the Keller Army Community Hospital (KACH), is to provide high quality, patient-centered care with a focus on health and wellness to improve readiness of the Force and enhance the lives of all beneficiaries. 3. REQUIREMENTS: The purpose of this RFI is to obtain potential vendors that can provide maintenance services identified in section four of this RFI. The KACH seeks technology that is able to provide compassionate, quality outpatient and preventive medical care to their beneficiaries and first echelon emergency medical support. Information regarding this request, should be provided in the form of a information paper and should also discuss related company experience and include point(s) of contact. The information paper should be prepared in MS Word, and submitted electronically to the contracting office to jared.l.goins.ctr@mail.mil and CW2 Kenneth J. McCutcheon at kenneth.j.mccutcheon2.mil@mail.mil. 4. DELIVERABLES: Maintenance Support Contract (Annual) for the Abbott Intralase FS (FemtoSecond) Optical Laser must meet the following specifications: 4.1 24/7 phone access to technical support personnel with response time not to exceed 48 hours from notification of failure 4.2 unlimited responses to unplanned equipment failures 4.2 guaranteed up time of 90% or better, with repair time not to exceed 96 hours 4.4 two scheduled services within the annual contract periods planned IAW MFG, requirements must include scheduled Original Equipment Manufacturer (OEM) parts only 4.5 costs for parts, travel and labor must be included within the contract with no third party labor, or subcontracting of any kind 4.6 must have authorization and capability to update system software 4.7 service technicians must be certified by the Manufacturer (Abbott) and have a minimum of 2 years servicing this type of equipment (Intralase Lasers) 5. Vendors will need to be able to provide item within 30 days of awarded contract.
