Description

This is sources sought announcement only. The purpose of this notice is to conduct market research and obtain information from qualified potential offerors. No formal solicitation document exists at this time. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The Durham VA Medical Center (VAMC), 508 Fulton Street, Durham, North Carolina is seeking potential sources capable of providing the development of learning collaborative and a quality improvement project based on smoking cessation for homeless Veterans. THIS IS NOT THE COMPLETE DESCRIPTION LIST: Contractor shall develop a Learning Collaborative "Integrated Care Smoking Cessation for Homeless Veterans" in order to train VA providers working in homeless programs throughout the Mid-Atlantic region to provide evidenced-based smoking cessation care to homeless Veterans, and the development of a Quality Improvement Project to examine the "SmokefreeVET" program. SmokefreeVET is a new SMS texting smoking cessation intervention that was developed through collaborative efforts between the National Cancer Institute (NCI), the VA Public Health National Prevention Program and the VISN-6 MIRECC. The contractor shall provide all items necessary for the learning collaborative kick off meeting, implementation plan, faculty orientation, expert panel meeting, team selection and enrollment, learning collaborative launch and close meetings, learning sessions, action periods, consultations, site visits, metrics, project evaluation and final report. The contractor shall provide all items necessary to develop the SmokefreeVET Quality Improvement Evaluation to include kick off meeting, Veteran recruitment, enrollment and orientation to the "SmokefreeVET Quality Improvement Evaluation", weekly planning committee meetings, Veteran interview development, scheduling, facilitation, summative report, debrief meeting and s recommendations, and implementation plan for improvement recommendations. Responses to this sources sought notice should include the following: Name and Address of Company; Business Size (large or small); Identify whether or not firm falls into any of the following socioeconomic categories - 8(a), HUBZone, Service Disabled Veteran-Owned; Provide a Brief Capability Statement of Qualifications and any other pertinent information, which demonstrates ability to meet the above requirement.
