- MATOC Task Order Proposal Form for FMATOC-13-R-0002 Wage Rate Determination for FMATOC-13-R-0002 SOW for FMATOC-13-R-0002, Basewide Door and Entry Work, dtd 4 Jun 13 THIS PROJECT IS OPEN TO KSARNG/KSANG APPROVED SMALL BUSINESS MATOC CONTRACTORS ONLY. Pre-Bid Meeting will be held on Tuesday, June 25, 2013 at 1:00 pm or 1330 hr at Bldg 760, CE Bldg, Conference Room, Forbes Field, Topeka, KS. MATOC proposals will be due NLT 2:00 pm, CST or 1400 hr on Tuesday, July 16, 2013. Please email or fax proposals to Linda Criss, Contracting Office, USPFO for KS at (785 274-1642 or email to linda.j.criss.civ@mail.mil Project conssits of the following: SOW Non-complex design/build - Building 2005 main entrance repair, modify or replace existing manual doors and entrance with automated openers to support handicap entrance and exit. All work constructed under this contract shall be in accordance with the Internationl Building Codes and Americans with Disabilities Act. Building 668, PMEL, east side center of building replace door, frame and side window. Use solid exterior door with vision lite and sidelight. Building 662, TMO/Supply, replace door and frame use electronic digital keypad hardware and replace dented siding. Use solid exterior door with vision lite Building 662, Hazmat Pharmacy, replace glass door. Use solid exterior door with vision lite Building 662, Power Vault in Avionics, replace double metal doors. Use solid exterior doors no vision lite. SPECIAL PROVISION: 1. The Special Provisions shall apply to the above described project, Forbes Field, KS. 2. Award will be based on Contractor's proposal. 3. The Contractor shall remove and dispose the demolished material. 4. Proposals should include multiple alternatives and will be selected on best value to the government. All components will be commercial grade and for exterior use where exposed. 5. Quantities must be field verified by Contractor prior to award. 6. Work is to be performed between 7:00 a.m. to 5:00 p.m., Monday through Friday. Work on weekends and holidays only with written permission. The MATOC task order proposal form and wage rate determination is posted.
