Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is W912JD-13-T-2002 and is issued as a Request for Quotation. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-66 and DFARS Change Notice (DCN) 20130522. The North American Industry Classification System is 5413800 and size standard is $14 million. This requirement is a Competitive Full and Open After Restrictions -- Total Small Business Set-Aside. SUBMISSION AND EVALUATION OF QUOTES: To assure timely and equitable evaluation of the quotes, offerors shall follow the instructions contained herein. Quotes shall be submitted as a single electronic package to the Primary Point of Contract email address not later than the date and time identified within this notice. The offer must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The responses shall consist of the Price Schedule (Excel Spreadsheet), Past Performance, Technical and fill in Provisions/Clauses. Offers shall be evaluated using Lowest Prices Technically Acceptable criteria in accordance with FAR 15.101-2 and 52.212-2. DATE AND TIME FOR SUBMISSION OF QUOTES: Quotes shall be received no later than 1 July 2013 at 2:00 PM EST. Email quotes to MSgt Patrick Nott at patrick.nott@ang.af.mil. Quotes sent by other means WILL NOT be accepted. Late quotes will be processed in accordance with FAR 52.212-1(f) Late submissions, Modifications, Revisions, and Withdrawals of Offers. WAGE DETERMINATION: Wage Determination Number 2005-2241 Rev 13, dated 06/13/2012, State: Maine, Area: Maine Statewide, is incorporated into this solicitation. Request for a Wage Determination for another location shall be submitted in accordance with FAR Clause 52.222-49 - Service Contract Act-Place of Performance Unknown. SYSTEM FOR AWARD MANAGEMENT (SAM). To be considered for award, prospective offerors shall have a current registration in SAM in accordance with DFARS Clause 252.204-7004, Alt I - Central Contractor Registration. Access the system online at https://www.sam.gov. PRICE SCHEDULE OF SUPPLIES/SERVICES: Quotes shall be submitted using the attached Excel Spreadsheet. CLIN 0001 - Quarterly Storm Water Sampling - Base Year Services, Non-personal: To furnish all labor, parts, materials, facilities, and transportation necessary to perform the services in accordance with the Performance Work Statement. Provide testing on six (6) samples for each compound once a quarter for a year; BOD, COD, Oil and Grease, Total Suspended Solids, and Propylene Glycol. QTY 24 EACH CLIN 0002 - Monthly De-Icing Sampling - Base Year Services, Non-personal: To furnish all labor, parts, materials, facilities, and transportation necessary to perform the services in accordance with the Performance Work Statement. Provide testing on three (3) samples for each compound approximately monthly for seven (7) months out of the year; BOD, COD, and Propylene Glycol. QTY 21 EACH CLIN 0003 - Quarterly Storm Water Sampling - Option Year 1 Services, Non-personal: To furnish all labor, parts, materials, facilities, and transportation necessary to perform the services in accordance with the Performance Work Statement. QTY 24 EACH CLIN 0010 - Monthly De-Icing Sampling - Option Year 4 Services, Non-personal: To furnish all labor, parts, materials, facilities, and transportation necessary to perform the services in accordance with the Performance Work Statement. Provide testing on three (3) samples for each compound approximately monthly for seven (7) months out of the year; BOD, COD, and Propylene Glycol. QTY 21 EACH F.o.b. Point - Destination. Delivery of testing materials and final reports shall be at the vendor's expense in accordance with the Performance Work Statement. Shipment shall be to the following address: 101 ARW/CE Environmental Management Office 106 Ashley Avenue, Suite 486 Bangor, ME 04401 PAST PERFORMANCE: Past performance information will be use as part of this solicitation in determining Contractor Responsibility in accordance with FAR Subpart 9.1. Offerors are to include a single typed sheet listing a minimum of three (3) past performance references of previous prime contractor awards made to the Offeror within the past three (3) years with a Federal, State or local government, or other commercial contract of similar scope as this requirement. The following information shall be included: Contract Number; Contract Amount; Service Provided; Delivery Schedule; Contract Agency Name and Address; and Contract Agency Point of Contract Name, Phone Number and Email Address. TECHNICAL: Validation of Qualifications. The contractor's laboratory shall maintain certification by the State of Maine to perform the required analyses. The contractor shall provide a copy of their Maine analytical certification with their quote. The contractor shall notify the Contracting Officer (CO) within 7 calendar days of changes in certification status. The contractor shall inform the CO of any non-compliance activity cited by the OEPA during the life of the contract. Quotations submitted shall contain - SOLICITATION NUMBER; COMPANY NAME; COMPANY ADDRESS; COMPANY POINT OF CONTACT NAME; COMPANY E-MAIL ADDRESS; COMPANY TELEPHONE NUMBER; ANY PAYMENT DISCOUNT TERMS; AND, ACKNOWLEDGEMENT OF ANY AND ALL SOLICITATION AMENDMENTS.
