- Performance Work Statement Please complete and submit this evaluation sheet to the issuing office along with the bid schedule. Please submit pricing on this document. Please sign and date at the bottom of the document and return to issuing office. Wage Determination This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, supplemented with additional information included in this notice. This announcement constitutes one (1) solicitation which will result three (3) Firm Fixed Price Indefinite-Quantity contract awards. Anticipated awards will be the result of one solicitation under Full and Open competition and two under Small Business set-aside conditions. However, the Government reserves the right to award less than the anticipated amount for either category. If no large business offers are received, only two awards will be made. Proposals are being requested and written solicitations will not be issued. Solicitation Number CAIB01-03 is hereby issued as a Request for Proposal. This combined synopsis/solicitation and incorporated provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-66, effective 1 April 2013. The North American Industry Classification System (NAICS) code for this project is721110, and Standard Industrial Classification (SIC) code 7011. The Size Standard for NAICS 721110 is $30M. The contractor shall provide all labor, equipment, materials, tools, parts, shipping, and all other items necessary to provide items defined in the Performance Work Statement. Offerors shall submit pricing for CLINS 0001 to 0004 (including options) for "peak season" and CLIN 0005 through 0008 (including options) "off season" timeframes. Peak season is defined as retreat dates that fall between the months of April through September. Off season is defined as retreat dates that fall between the months of October through March. Retreats are comprised of various commodities and services that may or may not be required for individual retreats, refer to the Performance Work Statement for a complete description of required services for each CLIN. Please provide pricing for the following base year and four (4) option years. The following provisions and clauses apply to this solicitation and will remain in full force in any resultant award: FAR 52.212-1, Instructions to Offerors - Commercial Items (Feb 2012) is amended to read: Offeror shall submit signed and dated offer to 1 SOCONS/LGCA, Attn: Cole Johnson, 350 Tully Street, Building 90339, Hurlburt Field FL 32544-5810, by 22 July 2013, 2:00 p.m. CST. Submit signed and dated Offer Schedule marked with solicitation number CAIB01-03. The offeror may submit an electronic copy of their signed and dated offer via e-mail to 1SOCONS.LGCA2@hurlburt.af.mil and cole.johnson@hurlburt.af.mil with the solicitation number in the subject block. The government reserves the right to evaluate proposals and award a contract without discussions with offerors. Offers shall be evaluated in accordance with FAR 52.212-2, Evaluation of Commercial Items (Jan 1999) which is incorporated into this Request for Proposal below. The Government anticipates the award of three or less contracts from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Technical capability (ii) Price. Technical capability is approximately equal to price. The Government will award a Firm Fixed Price - Indefinite-Quantity contract to the technically-acceptable and responsible offeror(s) with the lowest priced offer. The offeror shall submit a completed Offer sheet with pricing to be evaluated on the attached Evaluation sheet with proposed amounts for a retreat. Actual projects will solicited solely among the final contract recipients and issued as task orders to the original contract. Technical acceptability will be determined by the following: 1. Lodging must meet the specifications identified in the Performance Work Statement. Lodging includes the offerors proven capability to supply a minimum number of rooms in the proper room sizes (as identified in the offer schedule for price evaluation), amenities, and patron parking. Lodging rooms shall be in a centralized location where patrons are not subjected to the elements for more than 5 minutes while traveling to meeting space and childcare locations. Additional travel requirements such as shuttle/bus transportation may be offered free of charge and shall be at the will of the patrons and not require pre-arrangement. 2. Meeting space must meet the specifications listed in the Performance Work Statement to include a minimum of at least two (2) conference rooms. 3. Childcare must be provided in accordance with the Performance Work Statement. Space must be provided for children in a separate location from adults. 4. Meal choices/types must meet the specifications listed in the Performance Work Statement. Meals must be provided on-site. 5. Pricing must be provided for all specified CLINS to include the base year and four (4) option years. 6. Contract location must be within 70 miles of Hurlburt Field, FL. 7. Offeror CAGE Code shall be the CAGE code for the actual hotel/resort and/or hotel/resort parent company, LLC, or owner. The offeror may access the current replacement site for ORCA, the System for Award Management (SAM), at (https://www.sam.gov/portal/public/SAM/) 52.216-18 Ordering (Oct 1995) (a) Any supplies and services to be furnished under this contract shall be ordered by issuance of delivery orders or task orders by the individuals or activities designated in the Schedule. Such orders may be issued from the contract award date through the contract period of performance, including exercised options. (b) All delivery orders or task orders are subject to the terms and conditions of this contract. In the event of conflict between a delivery order or task order and this contract, the contract shall control. (c) If mailed, a delivery order or task order is considered "issued" when the Government deposits the order in the mail. Orders may be issued orally, by facsimile, or by electronic commerce methods only if authorized in the Schedule. (End of Clause) 52.216-19 Order Limitations (Oct 1995) (a) Minimum order. When the Government requires supplies or services covered by this contract in an amount of less than $10,000.00 the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract. (b) Maximum order. The Contractor is not obligated to honor -- (1) Any order for a single item in excess of $1,500,000.00; (2) Any order for a combination of items in excess of $1,500,000.00; or (3) A series of orders from the same ordering office within 30 days that together call for quantities exceeding the limitation in subparagraph (b)(1) or (2) of this section. (c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section. (d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 7 days after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source. (End of Clause) 52.216-22 Indefinite Quantity (Oct 1995) (a) This is an indefinite-quantity contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies and services specified in the Schedule are estimates only and are not purchased by this contract. (b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. The Contractor shall furnish to the Government, when and if ordered, the supplies or services specified in the Schedule up to and including the quantity designated in the Schedule as the "maximum." The Government shall order at least the quantity of supplies or services designated in the Schedule as the "minimum." (c) Except for any limitations on quantities in the Order Limitations clause or in the Schedule, there is no limit on the number of orders that may be issued. The Government may issue orders requiring delivery to multiple destinations or performance at multiple locations. (d) Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor's and Government's rights and obligations with respect to that order to the same extent as if the order were completed during the contract's effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after the expiration of the period of performance. 52.217-8 Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 10 days. (End of Clause) 52.217-9 Option to Extend the Term of the Contract (Mar 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 66 months. (End of clause) 5352.201-9101 OMBUDSMAN (NOV 2012) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (d) The ombudsman has no authority to render a decision that binds the agency. (End of clause) 5352.223-9001 HEALTH AND SAFETY ON GOVERNMENT INSTALLATIONS (NOV 2012) (a) In performing work under this contract on a Government installation, the contractor shall: (1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and (2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes. (b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract. (c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.
