Solicitation number VA260-13-Q-0083 is being issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-67, dated June 21, 2013. The North American Industry Classification system code (NAICS) is 333912 size standard of 500 Employees. This is a total small business set aside. All firms responding must be registered with the System for Award Management (SAM). Offerors? quotes shall propose a firm fixed price for all line items listed below, subject to the terms and conditions of this entire synopsis/solicitation. The location for delivery/performance is: Department of Veterans Affairs Boise VA Medical Center 500 W Fort St. Boise, ID 83702 Quotes must be received by July 3, 2013 at 4:00 P.M. MST. Quotes may be submitted via e-mail, mail or in person. Award resulting from this RFQ will result in a Firm-Fixed-Price (FFP) purchase order in accordance with Federal Acquisition Regulation (FAR) Part 16. DESCRIPTION: The Department of Veterans Affairs is requesting quotes removal and replacement of medical vacuum pumps and tank in the basement of building 67 on the Boise VAMC campus. Contractor shall provide all materials, labor, tools and equipment necessary to accomplish this work. Work shall commence within 30 days of the contractor?s receipt of Notice to Proceed issued by the government. Total duration of this project shall not exceed 5 business days from the start date provided in the notice. ITEM 0001 Remove existing medical vacuum system in Bldg 67 and install new duplex tank mounted industrial liquid ring water-sealed vacuum pump system in accordance with attached SOW. Qty: 1.00 JB SITE VISIT: A site-visit to view the existing system requiring replacement has been tentatively scheduled for Thursday, June 27 at 1:00 PM at the Boise VA Medical Center. Prospective contractors interested in attending the site-visit are encouraged to e-mail William.Blyth@va.gov for the specific meeting location at least one-day prior to the scheduled site-visit. OFFERS: Offers must contain the following to be considered responsive: (1) The solicitation number; (2) The name, address, and telephone number of the offeror; (3) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; (4) Terms of any express warranty; (5) Completed Price schedule and any discount terms; (6) A completed copy of the representations and certifications at FAR 52.212-3 ALT I; (7) Acknowledgment of Solicitation Amendments (if any); (8) A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. PERIOD FOR ACCEPTANCE OF OFFERS: The offeror agrees to hold the prices in its offer firm for at least 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation. QUESTIONS: Questions regarding this solicitation are to be submitted in writing via email to William.Blyth@va.gov. The deadline for questions is Friday, June 28 at 10 A.M. MST. No phone calls will be accepted. Answers to questions will be posted on Fed Biz Ops as required. Individual responses are not promised, and should not be expected. SOLICITATION PROVISIONS AND CLAUSES All solicitation provisions and clauses applicable to this acquisition are included in the attached solicitation document, VA260-13-Q-0996.
