Description

SOW Vendor Questionnaire W9 THIS IS A SUBCONTRACTING OPPORTUNITY SCOPE OF WORK LRJC2013-026 NURSE PRACTITIONER The Little Rock Job Corps Center, operated by Adams & Associates, Inc., seeks a Nurse Practitioner for 6 hours per week for the period of October 1, 2013 - September 30, 2014, with the option for three month extensions for on-site services (based on submitted fee schedule). The Center offers both residential and non-residential options in career skills, academic, and vocational training for young adults from 16-24 years old. Deadline for proposals will be July 19, 2013, 5:00 PM CST. All bids must be on your company's letterhead and include the current W-9/Vendor Questionnaire in your bid packet (attached). Please provide a copy of state licenses, professional degrees, certificate of insurance, and/or certifications as well as a resume.
