THIS IS A SOURCES SOUGHT NOTICE INTENDED FOR MARKET RESEARCH PURPOSES ONLY. There is no solicitation available at this time. No contract will be awarded from this announcement. The purpose of this Sources Sought Notice is to determine the availability and capability of the commercial market for Custom Prepacks. This notice is for planning purposes only, and does not constitute an Invitation for Bids, Request for Proposals, or Request for Quotations, or an indication the Government will contract for the items contained herein. This notice is NOT to be construed as a commitment on the part of the Government to award a contract, nor does the Government intend to pay for any information submitted as a result of this notice. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. Please indicate which North American Industry Classification System (NAICS) Code is most appropriate for this commercially available of the shelf (COTS) items is 339113 with a size standard of 500-employees, if not please make a recommendation. The Veterans Integrated Service Network-23 (VISN-23), Network-23 Contracting Office (NCO-23 Fargo, ND) is conducting market research to identify potential qualified sources to provide Custom Packs, this program previously was provided through VA National Acquisition Center Federal Supply Schedule (FSS) Service Schedule FSC 65, Part II, Section A "Medical Equipment and Supplies" under Special Item Number A-18 "Sets, Kits and Trays, Disposable". The purpose of this market research is to identify the best procurement vehicle to meet this requirement, which is in line with the procurement hierarchy prescribed in Veterans Affairs Acquisition Regulation (VAAR) 808.002. If interested please submit your response(s) to the following market research questions concerning your capabilities to provide Custom Pre-packaged "Sets, Kits, and Trays, Disposable" by the date and time listed below: a.Company Information: 1)Company Name: 2)Doing Business As (DBA): 3)Address: 4)Socioeconomic Category: 5)DUNS Number: 6)TAX ID Number: 7)CAGE Number: 8)Is your firm currently registered in the System for Award Management (SAM)? b.Geographic Coverage (delivery area): c.Points of Production: d.Prompt payment terms: e.Does your firm accept the Government purchase card up to the micro-purchase threshold? f.Does your firm accept the Government purchase card above the micro-purchase threshold? g.Foreign Items? h.Shipping Terms: 1)Time of Delivery? 2)Does your firm offer Expedited Delivery? 3)Does your firm offer Overnight and 2nd Day Delivery? 4)Would your firm accept the terms of F.O.B. Destination? i.Ordering Procedures? j.Warranty provisions? k.What information will be required by interested parties to respond to a potential solicitation? l.What is your required lead time? m.What is the shelve-life for custom prepacks? n.What environmental controls are required for storage of custom prepacks? o.How long can custom prepacks be exposed outside of these environmental controls? p.How are custom packs packaged for shipping? Are there special handling requirements? q.Does your firm place bar codes on the exterior packaging of prepacks for inventory management? r.Are custom prepacks produced in lots? If so, is it industry standard to maintain samples as control measures? s.Is it industry standard to carry liability insurance? Respondents shall furnish sufficient information necessary for the Government to conclusively determine capability of your firm to provide custom prepacks, past performance history and client contact information for the last three (3) years. At a minimum, responses shall include the following: Business name, business address, business website, business size status (i.e., SB, VOSB, SDVOSB, HUBZone SB, SDB, WOSB, LB), point of contact name, mailing address (if different from the business address), phone number and email address. The government is not responsible for locating or securing any information, not identified in the response. Interested parties must respond with capability statements which are due in person, by postal mail, or email to the point of contact listed below on or before July 8, 2013 by 12:00 PM (Central Standard Time in Fargo, North Dakota) at the Network-23 Contracting Office (NCO-23), 2101 Elm Street, Building 30 (04-S), Fargo, ND 58102-2417 or email at John.North@va.gov. DISCLAIMER AND IMPORTANT NOTES: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provided feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. CONFIDENTIALITY: No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
