- NORTHLANDS JOB CORPS Center 100A MACDONOUGH DRIVE VERGENNES, VT 05491 802-877-0149 REQUEST FOR QUOTATION CENTER PHYSICIAN Conditions: The Northlands Jobs Corps Center requests your quotation for Physician services described in the attached document "Center Physician Statement of Work" according to the November, 2012 'Desk Reference Guide: Job Corps Center Physician' under the following conditions. We request a per hour cost within the designated number of hours needed on center to be provided between 8AM and 1PM any weekday morning, except Wednesdays. The designated number of hours has been formulated at 4.4 hours per week for 48 weeks a year. Current quotes must be guaranteed for the period August 1, 2013 through September 30, 2013, after which an extension of this arrangement is anticipated. The Northlands Job Corps Center reserves the right to reject all quotes. We also reserve the right to reject any portion of the proposal. Federal funds are used to pay all bills incurred by the Northlands Job Corps Center. As a result, all applicable federal regulations will be in effect.
