Description
This attachment is the information on intent to Sole Source this requirement NOTICE OF INTENT TO SOLE SOURCE. The Centers of Disease Control and Prevention (CDC) announces its intent to negotiate a sole source award with Emory University, Rollins School of Public Health, Diabetes Training and Technical Assistance Center (DTTAC). The objectives of this contract are as follows: 1) The Contractor shall maintain the Diabetes Training & Technical Assistance Center (DTTAC) and provide the necessary support to coordinate, manage and facilitate all aspects of developing and maintaining the DTTAC and its deliverables to include the planning and development of diabetes programs, trainings, systems, learning communities, and communications that enhance the ability of states to make significant contribution to the diabetes and chronic disease priorities to improve health and welness. 2) The Contractor shall plan and develop competency-based interactive diabetes learning opportunities tailored to the new diabetes core package activities and supporting strategies. This is intended to enhance the ability of DDT project officers to deliver effective diabetes specific technical assistance to the states. Specifially, the Contract shall provide/perform the following: 1) The Contractor shall provide the necessary support to coordinate, manage and facilitate all aspects of developing and maintaining the DTTAC, and the planning and development of trainings, learning management systems, and learning communities that will enhance the ability of states to make significant contributions to our collaborative chronic disease prevention and control efforts. 2) The Contractor shall furnish all necessary personnel, facilities, supplies and equipment, as appropriate, to provide the Centers for Disease Control and Prevention's Division of Diabetes Translation with logistical and programmatic support for planning, implementing, and evaluating the DTTAC and its role in supporting state work that is supportive of Collaborative FOA and the integration activities that are encouraged by participating NCCDPHP Divisions. 3) The Contractor shall deliver of make available regular trainings and workshops for state staff, the DDT Team, and other chronic disease division programs as related to the planning, implementation, and evaluation of comprehensive diabetes prevention and control work that supports an integrate approach to preventing and controlling diabetes and other chronic diseases and associated risk factors-such as heart disease, physical activity and nutrition. 4) The Contractor shall develop an online learning module that provides a menu of training opportunities and experiences. 5) The Contractor shall also develop specific topic related for DDT Project Officers based on an assesment of the competency needs. This DDT Project Officer competency project includes an assesment phase key informant interviews with leadership, analysis of interviews, report and discussion of results with leadership, in-person facilitated discussion with project officers to solicit needs. This is followed by a product development phase based on the needs assesment, priorities, and available funds. 6) The Contractor shall rspond to requests from states throughout the year for specific training, consultation and/or technical assistance related to the accomplsihment of DDT's work. THis training and consultation may include topics such as coalition development and action planning (Diabetes Today), planning fundamentals (Planning fundamentals online toolkit), consolidation and collaborative strategic planning, partnership development, diabetes policy, leadership development, coaching for excellence and people management, group facilitation and consensus building, e-health and the use of social media, running effective and dynamic online meetings, evaluation and logic modeling (online trainings available), and provision of web cast trainings on select topics stated above. These trainings may require in depth knowledge experts and/or consultation by outside consultants, which the contractor will be asked to coordinate. CDC intends to negotiate this requirement with Emory University, Rollins School of Public Health, Diabetes Training and Technical Assistance Center under authhority of FAR 6.302-1 and HHSAR 306.302-1, Only One Responsible Source, and 41 U.S.C 253(c)(1). This notice of intent is not a request for competitive proposals. A determination to compete this procurement based on a response to this notice is soley within discretion of the Government. Interested parties who believe they possess the capabilities to satisfy this requirement. All responses should be received by July 24, 2013 at 3PM, Eastern Standard Time (EST).
