Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation F1C0RH3161A001 is being issued as a Request for Quotation (RFQ). All solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-66, 26 Jun 2013. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). The NAICS code is 238310 and the small business size standard is $14.0M. The following commercial items are requested: Read Statement of Work. *ALL ASSOCIATED SHIPPING COSTS MUST BE INCLUDED WITH FINAL QUOTE. SHIPPING/DELIVERY OF SERVICES/MATERIALS IS FOB DESTINATION TO GUAM: 554 RHS/DE OIC Andersen AFB, Guam 96542. Submit your quote via email to SrA Geoffrey Bender at geoffrey.bender.1@us.af.mil. Responses to this RFQ must be received via e-mail not later than 4:30 PM local time on Thursday, 11 July 2013. Oral Quotes will not be accepted. Collect calls will not be accepted. All firms must be registered in the System for Award Management (SAM) database at https://www.sam.gov/portal/public/SAM/ to be considered for award. All proposals must be directly submitted by the firm registered in SAM that is intended to be the successful awardee. Proposal submitted by another firm on behalf of a SAM registered company with the intention of being award "care of" will not be accepted. Award will be based on lowest price technically acceptable. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items is included in this solicitation, and the offeror must include a completed copy of this provision with their proposal or be currently registered in the SAM website with their Representations and Certifications completed. All quotes, questions, and any requests for more information must submitted in writing and be sent to SrA Geoffrey Bender at geoffrey.bender.1@us.af.mil.  STATEMENT OF WORK SAKW 08-1004- Commando Warrior Simulator Facility 1.0 PROJECT BACKGROUND 1.1 USAF Personnel constructed an approximately I 08 feet x 55 feet concrete tilt-up facility with a poured concrete roof at Northwest Field-Andersen AFB, Guam as part of the Commando Warrior compound for the PACAF Regional Training Center (PRTC). 2.0 SUMMARY OF REQUIREMENTS 2.1 The contractor shall provide all labor, eq uipment, supplies, and materials to install a ceili ng insulation system, based upon design requirements provided by the Engineer of Record (EOR). The system shall incl ude all vapor barrier, insulation, and fire barrier. Deliverables shall be coordinated with Contracting Office's Representative (COR) and are subject to the work schedule stated in this document. All work will be performed according to the International Organization for Standardization (ISO) 9001 2000 certified procedure. Installation of the ceiling insulation shall be completed by I Sep 2013. 2.2 Codes and Regulations: The Contractor shall ensure that all work, and all installations are accomplished by following the MOST RECENT Unified Facilities Criteria (UFC), International Buildi ng Code (IBC), International Mechanical Code, International Fire Code and National Fire Protection Association (NFPA) standards. The most st ringent code shall apply in all cases. Deviations from the codes can only be approved by the Contracting Officer in writing. In addition, all work shall be performed in a professional manner by journeyman craftsmen certified in the trade in which they are employed. 3.0 ARCHITECTURAL 3.1 Facility is approximately 108 feet x 55 feet. The facili ty is approximately 5940 square feet total. No interior columns. 3.2 Ceiling insulation system shall be spray-foam type i nsulation and shall be installed onto the underside ofthe existing metal ceiling panels. 3.3 Insulation shall meet R-19 thermal resistance. 3.4 Contractor shall appl y DC-3 I 5 fire barrier coati ng over all i nsulation. 4.0 SUBMITTALS 4.1 Contractor shall submit for the Contracting Officer's approval, catalog cuts for the equipment and materials intended for use in the facility. Finished color selections for ceiling insulation system must be approved by Contracting Officer prior to i nstallation. Contractor shall provide a complete inventory of all system components, pieces and parts as part of installation. Submittal items must be written in the English language.
