Description: Statement of Work (SOW) GENERAL. The 301st Operational Contracting at NAS JRB Fort Worth, Texas, intends to award a Firm-fixed Price contract under Simplified Acquisition Procedures (SAP). This is a Combined Synopsis / Solicitation (CSS) for a commercial item prepared in accordance with the format in FAR Subpart 12.6 and 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation (SF1449) will not be issued. The CSS number 301CONF - 1654 is issued as a request for quotation (RFQ). CSS 301CONF - 1654, incorporated provisions and clauses are those in effect through the Federal Acquisition circular (FAC) Fac 2005-66, effective 28 Feb 2013; Defense Acquisition Circular (DAC) DPN 20130228, effective 28 Feb 2013 edition; and, AFFARS Parts current thru AFAC 2012-1107, effective 07 Nov, 2012. RESTRICTIONS. This acquisition is set-aside for competitive participation by small business inaccordance with FAR Part 19. The North American Industry Classification System (NAICS) code is 423320, with a size standard of $14,000,000.00. REQUIREMENT : See attached Statement of Work (SOW). DELIVERY : FOB destination, 7705 Wagner Road, Mineral Wells, Texas 76067. SITE VISIT : IAW FAR 52.236-3 --¬ "Site Investigation and Conditions Affecting the Work." An organized site visit is scheduled and it is highly recommended that all interested parties attend. For site visit information, please contact Marvin Frizell, Contract Specialist, at marvin.frizell.1@us.af.mil. FUNDING STATUS : This acquisition is currently FUNDED. SUBMISSION OF OFFERS. (REF: 52.212-1 -" Instructions to Offerors -- Commercial Items"). Submitted quotes shall include the following: (1) the solicitation number; (2) the time specified in the combined synopsis/solicitation for receipt of offers; (3) the name, address and telephone number of the offeror; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; (5) Terms of any express warranty; (6) Net price and any discount terms; (7) "Remit to" address ( marvin.frizell.1@us.af.mil ); (8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically); (9) Acknowledgment of any Solicitation Amendments (if applicable); (10) estimated delivery/completion time; (11) Past performance information for similar projects, to include prior customers points of contact with telephone numbers and other relevant information; (12) Statement that the offeror agrees to hold the price in its offer for at least 30 calendar days from the date specified for receipt of offers;(13) Include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the combined synopsis/ solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. EVALUATION OF SUBMISSIONS ; IAW FAR 52.212-2 - "Evaluation - Commercial Items. The Government will award a contract utilizing "Low Price - Technicaly Acceptable" (LPTA) evaluation criteria.
