- Bid Schedule Worksheet Template Catholic Music Coordinator Combo Solicitation Combined Synopsis and Solicitation, Catholic Music Coordinator This is a solicitation for a commercial product; prepared in accordance with the format in FAR Subpart 12.6 and 13.5, streamlined procedures for evaluation and solicitation for commercial items- as supplemented with additional information included in this notice. Quotations are being requested and a written solicitation document will not be issued. Description: Contractor will need to bid on the following items as listed in Attachment 1, Bid Schedule, FA5000-13-T-0008 for a base year and three each 12 month option periods. A Pianist/Organist is preferred; however, ability to provide music via any musical instrument may be accepted at the Government's option. Also, the Government desires to award a consolidated contract with the Catholic Music Coordinator and Musician; but reserves the right to award two separate contracts if in the best interest of the Government. This would be a contract for the Catholic Music Coordinator and a separate one for the Musician services. All responsible Contractors shall provide a quote in accordance to the specifications stated above. Transportation costs shall be included in the quote. If bidding on other than both the Catholic Music Coordinator and Musician services specified in the specifications bidders Contractor shall submit their quote on company letterhead, delivery time, name, address, telephone number of the offeror, terms of any express warranty. Also, their unit and extended prices shall be submitted on the attached Bid Schedule provided in MS EXCEL format. Interested companies that meet the requirements stated above should submit a quote. Quotations for this notice shall be received by July 18, 2013 at 12:00 PM Alaska Standard Time. Quotations shall be submitted to: Ms. Viola Stepetin E-mail: viola.stepetin@us.af.mil Phone: 907-552-6660 The following provisions and/or clauses apply to this acquisition: 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical Ability and Qualifications Price This will be a lowest price technically acceptable purchase. (a) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (b) A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The solicitation document, incorporated provisions, and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-38. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 15 calendar days from the expiration of the contract. FAR 52.217-9 -- Option to Extend the Term of the Contract (Mar 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within 15 calendar days from the expiration of the contract ; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 calendar days from the expiration of the contract. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years and six (6) months. In order to be considered for an award, an offeror must have completed the online electronic Representations and Certifications located at http://orca.bpn.gov/ in accordance with FAR 4.1201(a). Potential contractors must also be registered in the Central Contractor Registry (CCR) at http://www.ccr.gov. This is best-value procurement. Award will be made to the offeror whose offer is conforming to the solicitation and is determined to be the most advantageous to the Government. Price and other factors will be considered. The Government intends to evaluate offers and award without discussion. Offeror shall submit all information with offer as required in FAR 52.212-1. Quotations shall be submitted to Viola Stepetin at viola.stepetin@us.af.mil. All responsible sources may submit a written proposal, which shall be considered.
