- STATEMENT OF REQUIREMENTS KCVA MINOR CONSTRUCTION - MODULAR SPECIALTY CASEWORK Introduction The request is a "Meet or Exceed" requirement to obtain casework for our existing hospital which is undergoing a renovation of 2.5 floors which includes a Pre/Post Surgery floor and a Dialysis Unit. This modernization of space will update patient care areas to include patient isolation rooms, dialysis care area, and nurse stations that will require replacement casework. For the ease of long term maintenance, the KCVA Medical Center has been transitioning to modular casework. This allows the hospital to replace parts of a fixed cabinet / headwall versus replacing the entire millwork and undergoing construction activity to address issues. *** Please, forward a capability statement of 3 or 4 past performances that your organization has completed that is similar to this procurement; the requested requirements are a must to be considered for this procurement*** The requested requirements consist of: 3 or 4 past performances that are similar to this procurement. Product Requirements 1.Modular Headwall/Caregiver Stations a)Should take up a minimum of floor space and is necessary to be wall mounted above the floor to facilitate cleaning and housekeeping. b)Must support the rail hanging components of the Integrated Modular Medical Support System. c)Horizontal Rails Wall Rail 1)Should provide horizontal interface capability to suspend horizontal and vertical hung components. 2)Offered in a minimum of four nominal widths of 24", 30", 48", 60", 72", 84" and 96". Please refer to the equipment drawings for specific sizes. 3)Material should be anodized aluminum. 4)Have available End Caps, Connectors, appropriate Wall Anchors and Anti-dislodgement clips. 5)Must be capable of supporting a static load of 250 pounds per linear foot. d)Vertical Rails Stiles 1)Rail Attached Stiles a.Available in two heights of 24" and 64". b.Can be mounted anywhere along wall rails. c.Supports all types of tiles. d.Fabricated of 18 ga. cold rolled steel with a powder coat finish. e.Creates a minimum space of 3" to accommodate plumbing, power, data and electrical connections. 2)Stiles a.Is 18" high and required for attaching Tile b.Should be 18 gauge steel with a black finish. 3)Canopy a.Must support Canopies that are 24", 36", 48" and 60" wide. b.Created of 18 gauge steel with a black finish. c.Have an available connector kit and lockdown accessories. e)Tile All tiles should have an overlapping ship-lap design at all horizontal joints to prohibit moisture from wicking behind tiles. Bed Tile 1)Available in nominal widths of 36" and 48", height of a nominal 31" and depth of.75 inches. 2)Designed to take the impact of a common patient bed without additional reinforcement. 3)Comes in at least three colors. Rotating Work Surface 1)Comes in 34" wide, length of 12" and.75" thickness and capable of supporting 150 pounds. 2)Must have a rotating arm support painted metallic silver. 3)Accommodates seated or standing heights. 4)Offered in at least three colors. g)Sink Modules 1)Comes with a nominal width of 24" and depths of 13" and 21" as well as an ADA version. Please see drawings for exact dimensions. 2)Available with a straight or curved integrated backsplash. Please see drawings for exact details. 3)Has offset drain to reduce backsplash. 4)Must have a 4" faucet that is AC/DC power and has an AC powered faucet sensor, hydro-powered faucet sensor or battery powered faucet sensor. 5)Offered with a sink enclosure that is 24" wide and mounts under the sink capable of supporting 150 pound load. 6)Sink enclosure material must be Durawrap to prevent moisture contamination. h)Cabinets Wardrobe Cabinet 1)Available at a nominal width of 24" and height of 63.5" and depths of 13" and 21". The kickbase, worksurface and bottom shelf is made of exterior grade plywood. f)The doors and outside panels of the unit must be of a high-grade laminated finished particle board. g)Non-porpous solid surfacing that does not support microbial growth "Beige" Countertop in a custom design (see drawings) h)Two Non-porpous solid surfacing that does not support microbial growth "Cream" Sinks on opposing "clean" and "dirty" sides i)Custom 4'-6"wide and 2'-3"high Non-porpous solid surfacing that does not support microbial growth "Beige" wave center backsplash that allows space for paper towel and soap holders to attach to it via a utility rail with clips j)A space allocated at the end of the unit on each side for the waste can k)Open plumbing chase on the interior of the cabinet l)Arc style hardware in a silver finish 3.Ambulatory Surgery Nurse Station / Dialysis Nurse Station / Dialysis Reception Desk To be compatible with the system furniture in the main facility for inventory and reuse in a variety of Medical/Administrative/Materials Handling purposes, the systems furniture/filing must meet the following: Frames a)Available in nominal heights of 38", 54", 70", and 86" and widths of 2' to 4' in 6 inch increments. b)Offered with worksurface heights that are 29 to 34 inches, adjustable in 1-inch increments to accommodate patients in wheelchairs. c)Should have hinged injection-molded or similar extruded PVC wire management baseboard covers accessible on both sides which provides locations for electrical outlets and an enclosed raceway for the non-electrical. The hinged covers should allow for the laying in of non-electrical and eliminate the need to thread wires through the base of the frame and around the corners. d)Includes metal connectors that are firmly secured to one another quickly using a simple hand tool (i.e. screwdriver or wrench) and allows for a two way 90 degree, two way 135 degree, three way, four way or straight line configurations. Frames must be secured so that light is not transmitted through the connectors. e)Designed to accept at least eight duplex receptacles per frame. A four foot wide by 38" frame must; however, accommodate at least eight duplex receptacles. The frames must have the capability of being wired with two modular eight wire, four 20 amp circuit electrical distribution systems of which one circuit should have the capability of being a completely dedicated circuit. The receptacles should be located at the baseline, vertically every 8 inches, and work surface level. The entire system must be UL listed. Please refer to the equipment and electrical drawings for specific requirements. f)Capability of receiving a hard wired (using 3/8" type MC aluminum cabling) solution with hospital grade receptacles. The frames must accommodate both emergency and non-emergency power in the same frame. The receptacles should be accessible at the baseline, vertically every 8 inches, and at the work surface. The electrical components must be UL listed. g)Includes a wire management system that permits both low voltage and data telecommunications cabling to be accessible at both sides of the frame at every 8" increments including the baseline and work surface levels. h)Frames designed to accept various sizes of access panels with different finishes and perform different functions (example is an equipment access panel). i)Frames of unequal heights should be able to connect to one another and capable of being anchored to the floor and with ability to be removed without disrupting adjacent frames. j)Designed to accept a variety of work surfaces, including work surfaces at 135 degree corners and counter tops with task and ambient lighting. k)Should accept support rails in five nominal lengths from 24" to 48" wide. These rails are used to support storage units. l)Available with stacking frame which will increase the height of an existing frame by a nominal 16" without removing the frame from adjacent componentry. The stacking frame must be constructed of a minimum of 16 gauge steel and must be load bearing. m)Offered with pre-attached baseline junction boxes for hard wiring to decrease electrical installation time. Access Panels (Please refer to the drawings for specific finishes and sizes.) a)Available with pre-attached baseline junction boxes for hard wiring to decrease electrical installation time. b)Offered in vinyl covered, laminate, painted, tool-supporting, fabric covered, wood veneer, marker board, acoustical (STC 26, NRC.80), glazed, tackable, open pass-through, beltline wire management, equipment support, equipment access versions and advanced cable management to accommodate fiber optics with a minimum bend radius of 7" at 90 degree corners, or a minimum of thirty-six 25 pair cables. Access panels for the wheelchair access module need to be fabricated from high-pressure.05 inch plastic laminate over 7/8-inch particle board core with hpl backer board on the underneath side. Offered in a finish that is produced by using the Powder Coated Process (epoxy resin paint with a metal substrate). c)Easily removable without the need for tools and permit nurse station equipment to be serviced from the outside of the nurse station without interfering with staff personnel utilizing the nurse station. d)The equipment access panel should extend the frames internal space by at least 6" to accommodate monitors and other medical equipment. It should be constructed of a minimal of.17" thick of high impact polystyrene or similar material which is UL approved. e)Ability to provide custom solutions with 3form in place of typical access panels. Countertops (Please refer to the drawings for specific finishes and sizes.) a)Available in Non-porpous solid surfacing that does not support microbial growth in all color options. b)Offered in custom styles, shapes, and sizes. Work surfaces (Please refer to the drawings for specific finishes and sizes.) a)Available in 24- or 30-inch depths and widths from 24 inches to 96 inches. Fabricated from high-pressure.05-inch plastic laminate over 1 1/2-inch particle board core with hpl backer board on the underneath side with extruded vinyl edge banding that has been adhesively applied. Hangs from both sides of the support structures and be adjustable up or down in 1-inch increments. b)Corner work surfaces should be available in at least two sizes. Fabricated from high pressure.05-inch plastic laminate over at least 1 1/8-inch particle board core with hpl backer board on the underneath side with extruded vinyl edge banding that has been adhesively applied. Hangs from both sides of the support structures and be adjustable up and down in 1-inch increments. The underside of the corner work surface should be predrilled holes for suspension of keyboard tray. c)Equipment support work surfaces and shelves should be available in 30- and 48-inch widths. They should be fabricated from high-pressure.05-inch plastic laminate over at least 1 1/8 - inch particle board core with hpl backer board on the underneath side. They should hang from any of the support structures and be adjustable up or down in 1-inch increments. They must extend through the frame to allow the support of extra deep equipment. An angled equipment shelf should be available to be recessed into an equipment support work surface to allow the monitors and equipment to be independently adjusted to the appropriate viewing angle of the user. d)Wheelchair access module work surface should be extended continuously across the open frame and has a depth of 34 to 40 inches and a width of 48 inches. The edges need to be rounded to assist in preventing injury to staff and patients. The work surface should hang from any of the support structures and be adjustable in 1-inch increments from 29 to 34 inches and must not extend beyond the countertop. Storage units (Please refer to the drawings for specific sizes and styles.) a)Manufactured in a variety of heights, depths, and widths and able to hang from both sides of the support structures and be adjustable up and down in 1-inch increments. Includes open shelves, closed storage shelves with lockable doors, patient chart shelves with adjustable dividers, lockable lateral files, and coat bar with shelves. b)A variety of pedestals that suspend under the counter, stationary and mobile, must be available to support the medical staff. The drawer fronts on the pedestals must be available in a variety of styles including one design that will support ADA compliance. c)Pass-through patient chart shelves with adjustable dividers must be available in 30-and 48-inch widths to allow access of patient charts from both sides of a structural frame. Storage cabinets (Please refer to the drawings for specific finishes and sizes.) a)Available in heights and widths to match the structural frames and in depths of 18 or 24 inches. Offered as lockable, freestanding, or wall integrated with connectors to hang or attach the cabinet to a frame. Doors are to be recessed into the cabinet and out of the way when open. They should come complete with leveling glides and adjustable organizing components to allow for the storage of paper, forms, coats, or other items. b)Large storage modules, approximately 2' wide x 1.5' deep x 6' high that can hold a variety of shelves and drawers, up to 9.5 cubic feet capacity, should be able to hang on the nurse station frames and be capable of becoming part of a supply transport system. This component should be manufactured from an injection molded thermoplastic or similar material with integral color throughout. An accessory such as an adapter rail should attach to the nurse station frame to provide for the horizontal interface to suspend horizontally hung components. The adapter rail needs to be available in lengths of 24" - 48" and made of aluminum or similar material that will support 800 lbs. and finished with a powder coat finish. Freestanding storage (Please refer to the drawings for specific finishes and size.) a)Available in combinations of other 6- and 12-inch-deep drawers. Offered in 19 5/8-inches deep and be fabricated from painted steel with either injection-molded plastic fronts, plastic laminate fronts, wood veneer fronts or painted formed metal fronts. b)Comes in widths of 24 and 48 inches, a depth of 19 5/8 inches, and two, three, or four-drawer high units. Must match the freestanding drawer pedestals in construction, front options, and finish. Organizing tools (Please refer to the drawings for specific types and sizes.) a)Available with a variety of organizing tools which hang from equipment support tiles. Options should include computer monitor arms, diagonal paper trays, horizontal and vertical paper trays, at least three sizes of form trays designed for healthcare forms, shelves, tackboards, tape dispensers, card files, document stands, telephone trays and message holders. Adjustable up or down in 1-inch increments without the use of tools. Electrical Service (Hard-wired electrical distribution system, hard wiring is not to be accomplished by the nurse station manufacturer but by the electrical contractor.) a)Where indicated on the drawings, the frames must accept a hard-wired solution with an UL listed raceway system. Installed and wired using 3/8" type MC aluminum by the electrical contractor. Providing receptacles at both sides of the frame at every 8" increment including baseline and work surface level as well as providing emergency and non-emergency power in the same frame. b)The electrical contractor is responsible for installing the 3/8" type MC aluminum cabling in the frames according to plans and elevations. Final connections to building power source is also the responsibility of the VA. Wire management a)Low-voltage, data telecommunications cabling and both types of electrical distribution systems must be accessible at both baseline and work surface levels. b)Wheelchair access module should provide for the continuity of wire management service across this module. c)Securely hinged baseline wire management side covers for easy access to cables and electrical; including receptacle and cable access covers. d)A minimum cross section of 12 square inches to accommodate monitoring cables and electrical conduit up to and including seven 3/4" or thirteen 1/2" diameter conduit with a minimum 7" bend radius at 90 degree corners in cable management tile. Miscellaneous a)Wall strips fabricated from cold rolled steel with a painted finish. Available in standard heights of 64" or 80" and allows the addition of hanging components that can be adjusted up or down in 1-inch increments. Adapter brackets available to allow wall strips to support a variety of surface access panels. b)Task lights must be UL listed, CSA certified and available in widths of 24 to 48 inches and should hang from the bottom side of both open and closed shelving units. They must not require the use of tools to install and be complete with a LED lamp, switch cord, and plug. Bumper Guard a)Available to be attached to the exterior of the nurse station at one (1") inch vertical increments to prevent damage to the nurse station access panels by wheelchairs, stretchers, material handling carts and other cart systems. b)Manufactured from high-impact vinyl/acrylic extrusions which are designed to act as a shock absorber under impact without damaging the nurse station to which it is mounted. The extrusion should have a matte finish, pebbled grain surface and be chemical and stain resistant and be UL Class 1 rating. The vinyl/acrylic guard must have a nominal wall thickness of 2.79mm securely locked over 50.8mm retainer clips positioned a maximum 24" on center.
