- RFP Attachment 2 - Past Performance Questionnaire Attachment 1 - Statement of Work (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulations (FAR), and supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a further written solicitation will not be issued. (ii) This is issued as a request for proposals (RFP). The RFP form is attached. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-68. Referenced FAR clauses can be found at www.acquisition.gov. (iv) This synopsis/solicitation is issued with full and open competition under the NAICS 334517 for "Irradiation Apparatus Manufacturing". (v) This RFP is for medium energy mobile imaging systems for the Non-Intrusive Inspection program. The attached Statement of Work provides details. (vi) Specific requirements are outlined in the attached RFP (HSBP1013R0051). The result of this RFP will be the establishment of an Indefinite Delivery Indefinite Quantity contract between the agency and the winning vendor. (vii) Orders will be requested via delivery orders on an as-needed basis. (viii) FAR 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. (ix) 52.212-2, Evaluation-Commercial Items is included. The contract will be awarded according to a trade-off analysis as outlined in the attached RFP document. (x) Please include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with any offer, unless already registered as active in the System for Acquisition Management (SAM) at www.sam.gov. (xi) FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. (xii) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items applies to this acquisition. Additional clauses cited by that clause are applicable and can be found in the attached RFP. (xiii) All other terms and conditions can be found in the RFP. (xiv) NO Defense Priorities and Allocations System (DPAS) rating has been assigned. (xv) Offers are due at 4:30 PM ET, August 7, 2013.
