Description
Additional Terms and Conditions IHS Average Daily Patient Load for the 28 hospitals 28 Hospitals and Bed Counts IHS 2013 Profile (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. (ii) Solicitation number 13-236-SOL-00037 applies, and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-68. (iv) The North American Industry Classification System (NAICS) code is 541611 with a small business size standard of $14M. This acquisition is 100 percent set aside for competition among small business vendors. (v) The Contract Line Items (CLINS) are as shown below. See the attached, Enclosure 1, Schedule of Prices for the details. The Contractor shall provide all labor, materials and equipment to perform the services for the event as indicated in the attached Performance Work Statement. CLIN 0001 - Completion of Objective 1, Total Firm Fixed Price CLIN 0002 - 28 Hospitals, Fixed Price CLIN 0003 - Travel (Per individual, as needed): Not to exceed 20 EA, not negotiable. Period of Performance: 1 Year (vi) The proposed order is for technical and consultative services to organize and generate an Inpatient Safety Performance and Organizational Assessment in IHS hospitals. The contractor will provide expertise to analyze currently existing processes and measures, improvement efforts for the requirement, identifying findings common to system hospitals as well as those unique to individual hospitals. The contractor will support the spread and development of solutions addressing common weaknesses and to provide onsite technical assistance to hospitals having unique needs. The contractor will support a measure-vention approach to onsite data analysis and improvement coaching. See the attached PWS for additional details. (vii) The delivery and acceptance term is FOB Destination. (viii) FAR 52.212-1 - Instructions to Offerors - Commercial Items (Feb 2012): Quotes shall be submitted on company letterhead stationery, signed and dated; it shall include: 1. Solicitation number. 2. Name, address, telephone number of the offeror and email address of the contact person. 3. Technical description of the item/service being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. See the attached, Enclosure 4, Quotation Submission Requirements. 4. Price and any discount terms. 5. "Remit to" address, if different than mailing address. 6. A completed copy of the representations and certifications at FAR 52.212-3. 7. A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. (ix) 52.212-2, Evaluation - Commercial Items (Jan 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. (3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law. (e) (1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c) and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items.
