- Data on exisiting equipment STATEMENT OF WORK FOR EMERGENCY CLEANING AT MALMSTROM AFB MONTANA DATE: 21 May 2013 PART 1- GENERAL 1.1 DESCRIPTION OF WORK: This is a pre-priced Blanket Purchase Agreement (BPA) that requires the contractor to provide all personnel, equipment, tools, materials, and services necessary to perform on-call emergency cleaning of all Malmstrom AFB facilities (not to include Missile Alert Facilities) to quickly remedy any condition (flooding, fire hazard, sewer backup, etc.) that poses a threat to personal safety or government property. All work performed on carpet will be in accordance with Engineering Technical Letter (ETL) 07-4: Air Force Carpet Standard. Services for this agreement shall be available 24 hours a day seven days a week. Services provided by the contractor shall include the following. 1.1.1 Water Extraction - The following types of water may need to be extracted as part of this BPA. • Clean water (water originating from a source that does not pose substantial harm to humans) extraction from broken domestic water supply lines, tub or sink overflows with no contaminants, appliance malfunctions involving domestic water supply lines, melting ice or snow, falling rainwater, broken toilet tanks /bowls that do not contain contaminants. • Gray water (water containing a significant degree of chemical, biological and/or physical contamination and having the potential to cause discomfort or sickness if exposed to humans) extraction from discharge of dishwasher/washing machine, overflows from toilet bowls with urine, sump pump failures, chilled and condensate water from HVAC, and fire sprinkler water. • Black water (grossly unsanitary water containing pathogenic agents, arising from sewage or other contaminated water sources that may cause sickness in humans) extraction from sewage and sewage/rainwater mix. All extracted water is to be legally disposed of using industry standards. 1.1.2 Anti-microbial Application - After water extraction the entire area that has been damaged by water will be treated with an anti-microbial treatment to prevent mold, mildew, fungus and bacterial growth from developing. All anti-microbial products shall not contain Ozone Depleting Materials listed on the EPA website. 1.1.3 Non-salvageable Removal - The contractor shall make a recommendation to the Government Inspector (GI) if it is determined that carpet; drywall, trim/base, and ceiling tiles need to be removed due to severe contamination or to prevent further contamination. The contractor shall remove these non-salvageable items only with approval of the Government Inspector and dispose of in the designated refuse container (See 1.3) 1.1.4 Drying - The contractor shall provide drying equipment required to remove excessive moisture from the damaged area. This shall include air movers and dehumidifiers designed specifically for the water damage restoration industry. 1.1.5 Prior To Cleaning - The Contractor will advise the GI of any conditions that prevent the carpet from being cleaned satisfactorily because of stains, fading, discoloration, or other conditions that may not be entirely removed by the accepted professional cleaning operations. 1.1.6 Post-Emergency Carpet Cleaning - The contractor is responsible for the removal of dust, litter, stains and soil from carpet. The contractor shall provide carpet cleaning as the last step on water damaged carpet. The carpet cleaning shall be completed using the hot water steam extraction method with a truck or van mounted unit. All carpet shall be chemically pretreated including spot removal prior to extraction. 1.1.7 Workmanship and Sanitation - Workmanship will conform to the generally accepted industry standards of quality of cleanliness, finish, freshness, and appearance. 1.2 UTILITIES: Water, sewer, electricity, and heat will be furnished by the Government at no cost to the contractor for accomplishment of work described in this Statement of Work (SOW). The contractor shall use Government furnished utilities in a prudent manner consistent with current energy conservation practices. 1.3 REFUSE CONTAINER: The Government will provide two 30-yard dumpsters located adjacent to Building 226 for the contractor's use to dispose of materials removed from facilities for accomplishment of work described in this SOW. 1.4 SAFETY: The Contractor shall comply with all existing Occupational Safety and Health Act (OSHA) standards of safety at all times in the performance of this BPA. Hazards to the safe use of the premises due to the contractor's work and equipment shall be suitably marked at all times. Pedestrian and vehicle traffic ways shall be kept clear and unobstructed. 1.5 WORK HOURS: Normal work hours will be 7:30am to 4:30pm, Monday through Friday. For the purposes of this BPA any work outside these work hours will be considered "After Hours". PART 2- EXECUTION 2.1 INSPECTION: There will be one GI and one alternate GI assigned to inspect for work compliance on each job (call) assigned. The GI will be the sole spokesman for compliance with the SOW. If the work is found to be unacceptable, the GI will inform the contractor of the objection part of the job and the contractor shall correct or re-perform any non-conforming aspect of the job. The CO will resolve any controversies that may arise between the GI and contractor. If a change in GI is required, the contractor will be notified in writing by the CO. The contractor shall coordinate all activities with the Operations Support Section (731-6978 or 731-6549). 2.2 CONTINUED USE OF FACILITIES: Occupants will continue to use their facilities during the clean-up process. The contractor shall notify the GI when a no-work condition will occur of more than one workday due to the functions of the facility. 2.3 FURNITURE MOVING: The building occupants will provide furniture moving when required. The contractor shall coordinate this with the Facility Manager. 2.4 ASSIGNMENT OF WORK: The GI will call the contractor for a work requirement covered by this BPA. The GI will provide the contractor with the Facility Manager's contact information. The contractor will contact the Facility Manager and coordinate the cleaning with the facility occupant's schedule. 2.5 WORKING IN THE WEAPONS STORAGE AREA (WSA): Strict security and personnel access control is maintained in the WSA. In order to gain entry into this area the contractor personnel are required to have an additional entry authorization letter (EAL) unique for this area and be physically escorted on site by authorized escort personnel. The contractor shall arrange for an escort with the owner agency when required to work in this area. 2.6 PERFORMANCE TIME: The contractor shall respond within two (2) hours once notified of a call unless otherwise directed by the GI. 2.7 INSPECTION NOTIFICATION: The contractor shall notify the GI once work is complete. The GI will inspect the completed job and accept or reject the service before the contractor submits an invoice for payment.
