- Amendment No. A00001 Solicitation No. VA259-13-R-0810 Provide Assisted Living Residence The following is a Q & A sheet for the referenced Solicitation: Q.Is a locked/secure unit required? A.No, a locked/secure unit is not required. Q.Is there an incumbent on an existing contract for this solicitation, or is this a new requirement? A.No there is no incumbent. Services are currently provided in-house. Q.Please provide an expected ALOS (average length of stay) for the residents referred by the VA. A.The ALOS varies, may be 3 - 14 calendar days. Q.1) "The Government shall order at least the quantity of supplies or services designated in the Schedule as the "minimum". Please clarify: what, or where is the "schedule referred to in this statement? 2) Does that mean the VA guarantees a minimum of 312 days per year? For the entire contract? If so, what does that translate to in terms of dollars? A.1) The "Schedule" refers to the Supplies/Services Prices/Costs, services to be provided, page 5. 2) Please remove clause 52.216-19, Order Limitations (1995), page 14 of the contract and replace with the attached. Q.Please define "Key Personnel". Who is considered a "Key" person to be identified as such in the proposal? A.Key Personnel refers to a point person of the contract facility familiar with the contract and familiar with the contract and care of the veterans who agrees to be readily available for admission and care coordination. Q.Information to be provided for each individual identified as "Key" person is to include: "Patient Safety Outcomes through Communication". What is this? What does this mean? Please clarify. A.Patient safety issues include AWOL, suicide attempts, psychiatric decompensation and/or conditions requiring emergent medical care for the veteran patient. Q.FAR 52.212-2, Evaluation - Commercial Items - 1 (Page 34). "What bearing does experience with an academic medical center have to do with this contract to provide AL services". A.This qualification item is hereby removed. ? Page 2 Amendment No. A00001 Q.Page 35, "Service Disabled Veteran-Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) Status". A,The decrease is for evaluation purposes only. There will be NO decrease on the actual amount paid to the successful providers. 1.A Sources Sought notice was posted on October 29, 2012. 2.The receipt of offers for this hereby extended an additional 4 weeks. The new receipt of offers date is August 15, 2013. End of Amendment No. A00001. ? 52.216-19 Order Limitations (Oct 1995) (a) Minimum order. When the Government requires supplies or services covered by this contract in an amount of less than 5 ALOS per contract period, the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract. (b) Maximum order. The Contractor is not obligated to honor -- (1) Any order for a single item in excess of 312; (2) Any order for a combination of items in excess of 312; or (3) A series of orders from the same ordering office within 365 days that together call for quantities exceeding the limitation in subparagraph (b)(1) or (2) of this section. (c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section. (d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 1 days after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source. (End of Clause)
