Description
Photos of gate motor Statement of Work Wage Determination (i) This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.603 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and written solicitation will not be issued. (ii) Request for Quote (RFQ) FA4897-13-Q-0006 is a combined synopsis/solicitation for commercial items. This combined synopsis/solicitation is issued as a RFQ. Submit written offers only, oral offers will not be accepted. All firms or individuals responding must be registered with System for Award Management (SAM). (iii) This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-67 and through Department of Defense Acquisition Regulation Change Notice 20130710. It is the contractor's responsibility to be familiar with applicable clauses and provisions. Clauses and provisions can be found at: http://farsite.hill.af.mil/vffar1.htm (iv) This procurement is being issued as a small business setiaside. North American Industrial Classification Standard: 811310; Small Business Size Standard: $7.0M CLIN 0001- Return two sliding chain-link gates to full operation to include ease of opening and closing, each gate should operate from a separate remote control, as well as manually, and have safety precautions put in place from closing on personnel or vehicles. Gate on Flightline side of compound is 40' wide and 6'8" tall. Gate on Road side of compound is 52' wide and 6' 8.5" tall. All repair work wiring must be sealed from the extreme Idaho winter and summer weather as well as rodents. Support posts will be used to reduce excess loading on the rollers and operating controls. Only metal products will be used in any repair work. Includes freight of materials to MHAFB, all site preparation and removal of debris. Install per statement of work (SOW). 1 each * See attached: Statement of Work (SOW) for description of work. 52.222-42 -- Statement of Equivalent Rates for Federal Hires. (May 1989) In compliance with the Service Contract Act of 1965, as amended, and the regulations of the Secretary of Labor (29 CFR Part 4), this clause identifies the classes of service employees expected to be employed under the contract and states the wages and fringe benefits payable to each if they were employed by the contracting agency subject to the provisions of 5 U.S.C. 5341 or 5332. This Statement is for Information Only: It is not a Wage Determination Employee Class Monetary Wage-Fringe Benefits 23370-General Maintenance Worker $16.45/hr (v) The government shall not reimburse respondents for any costs associated with submission of the information being requested or reimburse expenses incurred to interested parties for responses to this solicitation. The government reserves the right to cancel this solicitation, either before or after the closing date. (vi) Responsibility and Inspection: Unless otherwise specified in the contract or purchase order, the supplier is responsible for the performance of all inspection requirements and Quality Control (QC). (vii) The provision at FAR 52.212-1 Instructions to Offerors-Commercial Items, applies to this acquisition. SITE VISIT A site visit is scheduled for July 30, 2013 at 10AM local time. Access to Mountain Home AFB is required for all contractors. Pre-arrangement of passes must be coordinated through the listed point of contact (POC) if access is not already granted. All contractors entering Mountain Home AFB with commercial vehicles must do so at the commerical gate (Grandview Gate) located off grandview highway. Any interested parties can contact POC for directions. QUOTE/PROPOSAL INSTRUCTIONS: Offerors shall provide sufficient technical documentation (including but not limited to: product literature, specifications) to allow for thorough evaluation of proposal. Additionally, the vendor shall fully explain their ability to meet the requirements from above. Shipping shall be FOB Destination. Quote shall specify the Total Price. Quotes will be evaluated for Completeness (all line items are priced). Adequate competition is anticipated for this acquisition. The Government will award a firm-fixed price contract resulting from this combined synopsis/solicitation to the responsible offeror whose offer conforming to the synopsis/solicitation will be most advantageous to the Government, price considered. Offerors shall submit firm-fixed pricing for all above requirements. Include commercial price lists and if applicable, any FSS schedule numbers and pricing. Include any applicable discounts to the Government. (viii) The provision at FAR 52.212-2 Evaluation -- Commercial Items, applies to this acquisition. EVALUATION FACTORS FOR AWARD: The Government intends to award a contract resulting from this solicitation to the responsible offeror whose offer is the lowest price technically acceptable offer, which shall represent best value to the Government. Award shall be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors (i.e. technical conformance to the requirements of the solicitation). Evaluation of price will be based on the offeror's total price for all line items as delivered to the specified destination. Past Performance is not an evaluation factor for this acquisition. The following factors shall be used to evaluate offers: 1. LPTA Lowest Price Technically Acceptable; Price (all line items shall be priced) The offeror's initial proposal shall contain the offeror's best terms from a price and technical standpoint. The Government intends to evaluate offers and award a contract without discussions, but reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government reserves the right to make no award as a result of this solicitation. (ix) ) The clause at FAR 52.212-3 Offeror Representations and Certifications - Commercial Items, applies to this acquisition. An offeror shall complete only paragraphs (b) of this provision if the offeror has completed the annual representations and certificates electronically via https://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically at the System for Award Management (SAM) website, the offeror shall complete only paragraphs (c) through (o) of this provision.
