Disclosure to any third party without the written consent of the individual to whom the information pertains is strictly prohibited. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12, Acquisition of Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation that will be issued. The solicitation number for this requirement is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1, Simplified Acquisition Procedures. QUOTERS ARE ADVISED THAT AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE QUOTES RECEIVED. Quoters should not assume they will be contacted or afforded the opportunity to qualify, discuss, or revise their proposals. Therefore, the initial quote should contain the best terms from a technical and price standpoint. All quotes must include the full name, address, and DUNS number of the business to be named on the purchase order as well as the full name, phone number, and email address of the person who can accept the final purchase order. THIS IS A TOTAL SMALL BUSINESS SET-ASIDE. The North American Classification System (NAICS) code for this procurement is 562910 -Remediation Services- with a small business size standard of $19 Million. Quotes are due by Friday, August 9, 2013, by 2:00 PM EST and should be sent to the attention of H. Dale Bish via email to hbish@cdc.gov. Hard copies can be delivered via Fed Ex or UPS to H. Dale Bish, Centers for Disease Control and Prevention, Procurement and Grants Office, 626 Cochrans Mill Road, Pittsburgh, PA 15236. The Offeror must cite RFQ Number 2013-62009 in the email subject line and include it on the shipping label if sending hard copies. Evaluation and award of the purchase will be performed in accordance with applicable sections of FAR Part 12, Acquisition of Commercial Items, and FAR Part FAR 13, Simplified Acquisition Procedures. The Offeror must have an active registration with the System for Account Management (SAM), which replaced the Central Contractor Registration (CCR), prior to award of the purchase order in accordance with FAR Clause 52.204-7. The government intends to issue a Firm Fixed Price purchase order in accordance with FAR Part 13. Proposals must be submitted in two parts: (1) Technical Proposal and (2) Pricing Proposal. The Government will award a contract resulting form this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Technical and past performance, when combined, are significantly more important than price. EVALUATION CRITERIA 1. Technical Approach: In preparing the technical response portion of the task order proposal, the Contractor shall provide narrative and other information that is considered relevant to the work required to provide the services identified in the Statement of Work (SOW) and that best demonstrates the Contractor's technical capabilities and innovative approaches in those areas. The Contractor shall specifically address aspects such as technical understanding, methodology and approach, along with the sub-factors listed below. a. Provide a detailed statement of the problem, scope, and purpose of the project to demonstrate complete understanding of the intent and requirements of the contract and potential problems that may be encountered. b. Describe the proposed technical approach to comply with each of the requirements specified in the SOW. The proposal must be consistent with the stated goals and objectives. The proposed approach must ensure the achievement of timely and acceptable performance and illustrate a logical sequence of proposed events. c. Discuss the plan and solutions for overcoming difficulties involved with performing the work requirements. 2. Management Plans: The Contractor shall demonstrate an overall understanding of the project and adequacy and feasibility of plans to address all items in the SOW. This includes the detailed description of specific tasks to be performed, methods and resources (not staffing) to be used and a discussion of problems likely to occur and plans for addressing them. a. Contractor shall adequately describe the resources they propose to complete the work described in the SOW and a rationale for their proposed approach. The Contractor shall identify any resources that would be required above and beyond that already addressed as part of this proposal. b. The Contractor shall clearly indicate how the metrics in the SOW will be monitored and corrective action taken if appropriate. The Contractor shall specify its approach for maintaining control of all contract change management issues. 3. Similar Experience: The Contractor shall provide a description of three (3) projects completed within the past three years that clearly demonstrates the Contractor's experience in performing projects of similar scope, size and complexity to the requirements described in the proposal. The following information shall be provided for each project reference: a. contract number, customer/agency name and contract title; b. brief narrative description of the work performed for each of those contracts, including a discussion of any problems encountered/corrective actions and significant accomplishments; c. dollar value, contract type, period of performance, place of performance, and the number and types of personnel used in the performance of the contract; and d. name, address and phone number of at least two (2) customer contacts (Contracting Officer and Project Officer) for each of the identified contracts. Price Evaluation: A price analysis of the proposal shall be conducted to determine the reasonableness of the Contractor's price proposal Questions must be submitted via email to H.
