- VETS100 Form Bidder's Qualification Form FP03_Errata FP03_Eng Cert Prog for Bridge Temp Works (FHWA) Soils and Foundation Report Plans Bid Opening Date is scheduled for August 15, 2013 at 2:00 pm EST. This project is for only Certified Small Business Concerns Sealed bids from Certified Small Business Concerns only will be accepted for the Land Between the Lakes National Recreation Area Project KY ERFO FSR 2011-1(1), Solicitation Number DTFH71-13-B-00041, located in Lyon and Trigg Counties, Kentucky. The project consists of the reconstruction of a flood damaged bridge and roadway sections of Newby Cemetery Road [Forest Service Road (FSR) 130], Demumbers Road (FSR 108), and Energy Lake Road (FSR 134) within the Land Between the Lakes National Recreation Area. The bridge work includes the removal of the existing two-span, concrete bridge and the construction of a single-span, precast, prestressed concrete slab bridge with steel H-piles and structural concrete. The roadway work includes the installation of a 12-foot by 8-foot and a 12-foot by 10-foot reinforced concrete box culvert with earthwork, aggregate base and surface courses, hot asphalt concrete pavement, riprap, and other miscellaneous work. The project is expected to fall within the price range of $500,000 to $1,000,000. Contract Award/Notice to Proceed is subject to receipt of permits. Bid documents should be issued on or about July 25, 2013. Bid due date and location will be specified on the SF 1442, Block 13a of the Solicitation. Eastern Federal Lands Highway Division is issuing a combination of the synopsis and solicitation, the bid due date will be reduced based upon this. All bid documents can be directly downloaded from these websites. Documents will no longer be mailed. Register to receive Email Notification to automatically be notified when a document is added or updated for a specific project. If firms do not register as an interested vendor at the Federal Business Opportunities website, there will be no Plan Holders List. Annual Representations and Certifications FAR 52.204-8 (Jul 2013). The Representations and Certifications must be filled-in online at http://sam.gov/. The required Annual Form Vets-100 must also be filled-in online at https://vets100.dol.gov per FAR 52.222-37. The DOT's Short-Term Lending Program (STLP) offers lines of credit to finance accounts receivable. Maximum line of credit is $750,000 with interest at the prime rate. PLEASE NOTE For security reasons, individuals requiring access to the EFLHD must present a valid photo ID and be escorted to their destination by a Government employee. Due to new security provisions, all visitors attending bid openings are urged to arrive at least 1 hour prior to the scheduled bid opening. All visitors must register with the receptionist in Room 100. A Government employee will collect the bids. Prior to bid opening, a Government employee will escort all bidders to the bid opening. Unescorted visitors will be denied entry and no exceptions will be made.
