- THIS IS A SMALL BUSINESS SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs, Decatur GA, is conducting a MARKET SURVEY to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. Small business concerns should be capable of furnishing all labor, supervision, and parts, to include all equipment necessary to perform all work required under the proposed contract. DESCRIPTION OF SERVICE: Contractor shall lab pack, label manifest, and transport all waste on pickup date. Contractor shall coordinate pickup date with Hazardous Waste Manager (HWM). A waste hazardous materials inventory will be sent via e-mail for review and discussion. Contractor shall furnish all packing materials to include lab packs, drums, manifest, labels, and drum dolly. Contractor shall arrive at the Augusta VA Medical Center before 10:30 AM or after 1:00 PM and finish packing by 3:30 PM unless otherwise approved by HWM. Contractor shall provide HWM with a Certificate of Disposal (CD) within 30 days past transport date. Contractor shall be available for consultation regarding hazardous materials. Contractor shall be $2 million insurance. Contractor shall have vehicles that are road worthy and display all proper placards and warning sign. Contractor employees shall be properly trained regarding handling of hazardous materials. Contractor shall allow HWM access to staging area if requested. NAICS: 562112 POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Submit to: Department of Veterans Affairs, 500 Greene St, Suite 2, Hatcher Building, ATTN: Shirley Adams, Augusta, GA 30901. Responses should be received no later than 2:00 PM EDT, July 31, 2013. Submissions must be made via email to: Shirley.Adams@va.gov or FAX: (478) 274-5588. AT THIS TIME NO SOLICITATION EXISTS. (DO NOT REQUEST A COPY OF THE SOLICITATION). A FUTURE NOTICE WILL BE POSTED WHEN THE SOLICITATION WILL BE RELEASED. 4) Disclaimer and Important Notes: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. (5) No Proprietary, classified, confidential, or sensitive information should be included in your response.
