This is a Pre-solicitation Notice the solicitation posting date is anticipated to on or about July 29 2013. PERFORMANCE WORK STATEMENT FIRE EXTINGUISHER SERVICE SLC VAMC 1. DESCRIPTION OF SERVICES. 1.1. The contractor shall provide all labor, spark and explosive proof tools, equipment, test equipment, supplies, materials, parts, personal protective equipment (PPE) and clothing including reflective clothing (belt, vest, etc), hearing and eye protection including prescription safety glasses, lifting equipment and devices, transportation, and other incidentals necessary to provide Portable Fire Extinguisher Services for the Salt Lake City Veterans Administration Medical Center (SLC VAMC). Services are to be provided in accordance with the National Fire Protection Association (NFPA) 10 Standard for Portable Fire Extinguishers, Occupational Safety & Health Administration (OSHA) Code of Federal Regulations (CFR) 1910.155-157, Department of Transportation (DOT) Code of Federal Regulations (CFR) 49, and this Performance Work Statement (PWS). See Appendix A-1 1.2. Annual Scheduled Maintenance Services. 1.2.1. Annual Maintenance Inspection. The contractor shall provide the annual maintenance inspection in accordance with National Fire Protection Association (NFPA) 10 Standard for Portable Fire Extinguishers (in entirety), CFR 1910.155-157(OSHA) and CFR 49 (DOT) and this Performance Work Statement (PWS). 1.2.1.1. Annual Maintenance Service Point of Contact (POC).The contractor shall provide, upon contract award, the name and telephone number for contractor point of contract for annual scheduled maintenance service to COR. 1.2.1.2. Annual Maintenance Inspection Schedule. Annual Maintenance Inspection shall be scheduled within ten (10) business days after award of contract. 1.2.1.2.1. The contractor shall provide a legibly written copy of the Annual Maintenance Inspection schedule to the COR within ten (10) business days after award of contract. 1.2.1.2.2. Completion of Annual Maintenance Inspection. Annual Maintenance Inspection services shall be completed within forty-five (45) business days after contract award 1.2.2. General Occupancy Environment. While in each area conducting annual maintenance inspection on the portable fire extinguishers the contractor shall inspect the general occupancy environment relating to the portable fire extinguishers in accordance with NFPA 10 Standard. The contractor shall inspect for changes in building status that might affect the placement, type or quantity of portable extinguishers required. 1.2.2.1. The contractor shall identify recommended changes involving placement, type and quantity of portable extinguishers required to the COR. 1.2.3. Repairs and Recharge. The contractor shall provide repairs, as necessary, to render each extinguisher compliant with the Annual Maintenance Inspection requirements. 1.2.3.1. This may include: replacing parts, re-charge, de-charge, ensuring wall hanger is properly placed and secured, or installing a new hanger in compliance with NFPA 10 Standard. 1.2.4. Six-Year Maintenance. The contractor shall perform the Six-Year Maintenance Inspections in accordance with NFPA 10 Standard. 1.2.6. Disposal of Replaced Extinguishers. The contractor shall remove and properly dispose of all replaced extinguishers. 1.2.6.1 Prior to transporting off SLC VAMC the contractor shall ensure that all government tags, labels, or other identifying notations are removed or obliterated from each extinguisher. 1.2.6.2. Disposal Record. The contractor shall provide a legible written hand receipt to COR before taking fire extinguishers off the SLC VAMC. 1.2.6.3 Disposal Hand Receipt shall be in contractor devised format indicating the total number of fire extinguishers for disposal, manufacture, model number, size, serial number, contractor's name, name of technician, and date of service. 1.2.7. Annual Site Survey and Inventory Database (Electronic Version). The contractor shall provide an Annual Site Survey and Inventory Database (Microsoft Excel) to the COR no later than ten (10) business days after completion of annual maintenance inspection services. 1.2.7.1. The following minimum data shall be included on Annual Site Survey and Inventory Database: Location of the extinguisher (Bldg, post/room or directions); Identifying information for the unit to include: Manufacturer, Model Number, Size, Agent, Serial Number, Date Unit Manufactured; Date of last Six-Year Recharge; Date of next Six-Year Recharge; Month and Year Annual Inspection completed. 1.2.8. Semi-Annual Site Survey and Inventory Database (Electronic Version) The contractor shall provide a Semi-Annual Site Survey and Inventory Database (Microsoft Excel) to the COR no later than six (6) months after completion of annual maintenance inspection services. 1.2.8.1. The contractor shall include contractor's consolidation of Semi- Annual Site Survey records; Annual Site Survey and Inventory Database records; and Unscheduled Fire Extinguisher Maintenance service records for period of six (6) months following Annual Maintenance Inspection service completion date. 1.2.8.2. The following minimum data shall be included in Semi-Annual Site Survey and Inventory Database: Location of the extinguisher (Bldg, post/room or directions); Identifying information for the unit to include: Manufacturer, Model Number, Size, Agent, Serial Number, Date Unit Manufactured; Date of last Six-Year Recharge; Date of Next Six-Year Recharge; Month and Year Annual Inspection completed. 1.2.9. Records. The contractor shall provide and affix to each compliant portable extinguisher the appropriate maintenance records per NFPA 10, Chapter 6, Inspection, Maintenance, and Recharging. 1.2.9.1. The contractor shall provide the type Annual Maintenance Inspection Tag that provides space for the government's shop monitors to record the Monthly Inspection Record on the reverse side. 1.3. Unscheduled Fire Extinguisher Maintenance Service 1.3.1. Unscheduled Fire Extinguisher Maintenance Service Notification Process. After the initial inspection has been completed, when a fire extinguisher requires repair, re-charge, replacement of missing inspection/maintenance records or correction of non-compliance issues noted during the government monitor's monthly inspection or upon discharging an extinguisher, the COR shall by placing a telephone call to the designated contractor service point of contract (POC) provide a brief description of the problem, the name and phone number of government point of contact (POC) for the area the fire extinguisher resides. 1.3.2. Unscheduled Fire Extinguisher Maintenance Service Point of Contact (POC) Upon contract award, the contractor shall provide the name and telephone number for obtaining unscheduled fire extinguisher maintenance service to COR. 1.3.3. Unscheduled Fire Extinguisher Maintenance Service Response Time. The contractor shall respond to the COR request for assistance via telephone notification within 24 clock hours after receipt of notification. 1.3.3.1. The contractor point of contact (POC) and COR shall mutually agree to a date and time for the contractor's representative to arrive at SLC VAMC to perform the maintenance service requested. The agreed upon date shall be within 72 clock hours or less from the time of initial notification. 1.3.4. Completion of Services. The contractor shall complete requested maintenance service the same day the contractor arrives at the SLC VAMC. If completion of requested maintenance service is not possible the same day, services shall be completed on a mutually agreed upon date by the contractor and COR. 1.4. Maintenance Service Reports. 1.4.1. Annual Maintenance Inspection Report. In addition to the Annual Site Survey and Inventory Database (Electronic Version), the contractor shall provide to the COR upon completion of the Annual Maintenance Inspection, a report in contractor devised format. 1.4.1.1 The Annual Maintenance Inspection report shall include the following minimum information: Name of Company, Annual Maintenance Inspection service dates (from/to), total number of fire extinguishers provided annual maintenance inspection, number of fire extinguishers replaced. 1.4.2. Unscheduled Fire Extinguisher Maintenance Service Reports. The contractor shall upon completion of each unscheduled fire extinguisher maintenance service provide a legibly written service report to the COR. 1.4.2.1. Unscheduled fire extinguisher maintenance service reports shall be in contractor devised format containing the following information, as a minimum for each extinguisher serviced: Serial Number, Location, what types of Service and/or Repairs provided, Name of Technician and Date of Service. The service report shall be signed by the COR. 1.5. Parts and Materials. The contractor shall provide and use only new parts and material from the Original Equipment Manufacturer (OEM) in performance of this Performance Work Statement (PWS) unless authorized by the Contract Officer (CO) who will seek guidance from the COR. 1.6. Availability of Equipment. Equipment shall be available for maintenance services Monday through Friday, 7:00 a.m. until 3:00 p.m., excluding Federal Holidays, unless otherwise directed by the Contracting Officer (CO). 1.10. Applicable Laws. The contractor, in the event that applicable laws or regulations change during the terms of this contract, shall be required to comply as laws come into effect. 1.11. Contractor Compliance. The contractor shall comply with all applicable directives cited in this Performance Work Statement (PWS). 1.12. Housekeeping. The contractor shall keep the work areas clean and neat in accordance with (IAW) industry Standards, Occupational, Safety and Health Administration (OSHA), fire and safety standards and this Performance Work Statement (PWS). 1.14. Safety. The contractor shall also comply with all safety practices posted in the assigned work area and the attached Appendix C, provided by the Safety Office. 1.16. Certification. Contractor shall be a State of Utah licensed portable fire extinguisher company and certified to perform portable fire extinguisher services in accordance with NFPA 10 Standard. 1.16.1. Upon contract award, the contractor shall provide to the COR and Contracting Officer (CO) a copy portable fire extinguisher company state license and qualified agent certifications. 1.16.2. Contractor and Employee license certification records shall be made available to the Contracting Officer (CO) or COR or other authorized government personnel designated in writing by Contracting Officer (CO). 1.17. Contractor Employees. All contractor employees shall conduct themselves in a professional, courteous manner. 1.18. Privately Owned Motor Vehicles. Contractor personnel shall comply with directives pertaining to operation of privately owned vehicles on VA Property. 1.19. Security Requirements/Identification Badges. The contractor shall provide the name(s) of all contractor personnel requiring entry authorization to the SLC VAMC for performance of services associated with this requirement within five (5) business days after award of contract to Contracting Officer (CO) and COR. 1.20. Foreign National(s). The contractor shall notify the COR before sending a Foreign National representative to perform services. Special rules apply for foreign national(s) visiting a US Government facility. 1.21. Access Card. 1.21.1. The government will provide a "Identification Credential" which shall be issued, displayed and surrendered to the SLC VAMC Security Police. 1.21.2. The contractor shall ensure that all employees have the proper identification credentials prior to entering the VA Medical Facility. 1.21.3. The contractor shall provide the COR with copies of the turn-in receipts for and VA Identification Cards "Identification Credential" when no longer required for contractor employee. 1.24. Government Acceptance. Designated Contracting Officers Representative (COR) will accomplish inspection and acceptance of services performed based on requirements of the PWS, the contract, the 'Inspection Clause' and the Quality Assurance Surveillance Plan (QASP). 3. GOVERNMENT-FURNISHED PROPERTY AND SERVICES. Government Furnished Property (GFP) is not applicable to the performance of this contract. 3.1. Points of Contact. The government will provide a point of contact for each organization requiring fire extinguisher support services. The point of contact will escort contractor personnel to all fire extinguisher locations within their areas, as necessary. 3.1.1 The government point of contact POC will escort the contractor to ensure access to locations situated in designated "Controlled Areas" and the government escort will remain with the contractor at all times. 3.5. Workspace. The government will provide adequate workspace around the equipment area. 3.6. The government will furnish existing services located in the work area. Contractor shall use due care and diligence in efforts to conserve utilities to reduce utility costs. 3.7. Refuse Collection. Refuse shall be taken to the nearest authorized refuse dumpster for trash collection. Do not use the government dumpsters for collection of construction debris generated by this contract.
