- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is VA118-13-Q-0462 and is issued as a request for quote. Quotes are due to be in receipt by this office not later than July 31, 2013 at 12:00 PM EST. Award shall be made to the offeror that submits the lowest price that meets the requirements. Offerors must address each item in their quote that was addressed in the solicitation in order to meet the requirement. All vendors interested in submitting a quote for consideration should do so, prior to the solicitation closing date, via e-mail to Angel Santos at angel.santos2@va.gov and James Cozzolino at james.cozzolino@va.gov. This requirement is set aside 100% for small business and the North American Industry Classification System (NAICS) Code is 541519 with a corresponding Small Business Size standard of $25.5 million. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The mission of the Department of Veterans Affairs (VA), Office of Information & Technology (OIT), and VHA Department of Research & Development is to provide benefits and services to veterans of the United States. In meeting these goals, OIT strives to provide high quality, effective, and efficient Information Technology (IT) services to those responsible for providing care to the Veterans at the point-of-care as well as throughout all the points of the veterans' health care in an effective, timely and compassionate manner. VA depends on Information Management/Information Technology (IM/IT) systems to meet mission goals. The Cooperative Studies Program Coordinating Center (CSPCC) is committed to quality cooperative research studies that advance the health and care of Veterans through a continuous quality improvement system that produces innovative, efficient, safe and effective solutions to national healthcare problems. Several different software applications are essential to the conduct of these research studies. 1.REQUIREMENTS: The purpose of the effort is to procure additional brand name FaxBack Enterprise Fax Port licenses and software support. The continuous support services are needed to maintain the FaxBack Fax Port licenses currently installed in the FaxBack Dell Fax Server supporting CSPCC studies. The FaxBack Dell Fax server is running with FaxBack NET SATISFAXTION Enterprise Edition software and the AudioCodes devices is a gateway device to route fax calls to the fax server. This Fax Server is receiving hundreds of study forms faxes each day from study sites all over the United States. A prolonged outage of any sort would negatively impact the progress of many on-going CSPCC studies. The 12 month Annual Fax Port license software support shall include technical support from 6:30am to 5:00 pm M-F; except federal holidays, On-Line Knowledge base access, software and firmware updates. The proposed action is on a firm-fixed price basis for the procurement of additional FaxBack Enterprise Fax Port licenses and software support. Contractor cannot make any changes to the delivery schedule at the request of Site POC. Contractors must coordinate deliveries with Site POCs before shipment of hardware to ensure sites have adequate storage space. All shipments, either single or multiple container deliveries, will bear the VA Purchase Order number on external shipping labels and associated manifests or packing lists. Commercial license agreements may be made a part of this Contract but only if both parties expressly make them an addendum.
