- The Department of Veterans Affairs, Network Contracting Office 7, intends to award a one year, and four (4) option years, sole-source contract to procure annual maintenance service from Steris Corporation for Steris equipment currently utilized at the Ralph H. Johnson VA Medical Center. The contractor supporting preventive maintenance and Emergency repair services of sterilizing equipment shall provide: 1. For All Visits by Contractor: For repairs or services that will be performed during normal working hours, the Contractor (or Representative) shall check-in at the Biomedical Engineering Section office, Bldg. 1, Room G200, telephone (743) 789-7396. Contractor shall also notify biomed when leaving. Failure to follow these procedures may result in delay of payment or nonpayment. For repairs or service required after normal working hours, the Contractor is required to report to the Police & Security office at the main entrance to obtain clearance. Contractor is required to contact biomed to notify them of the service performed. 2. Preventive Maintenance (P.M.): a. Preventive Maintenance (P.M.) shall be performed by a factory trained service representative/service technician. These service individuals must be factory trained on all items listed in the equipment schedule. Documentation shall be provided to the Contracting Officer with the bid certifying that the service representative or service technician has been trained and certified in the performance of this work by the equipment manufacturer. b. All P.M. service shall be performed during normal V.A. working hours of 8:00 a.m. to 4:30 p.m. (deviations must be approved in advance by Contracting Officer Technical Representative). c. P.M. inspections shall be made in accordance with the original equipment manufacturers recommended schedule. During the P.M. visit, the contractor shall to provide all labor, supervision and parts necessary to correct any equipment malfunctions required prior to performing preventive maintenance, and assure that the equipment meets manufacturer's factory specifications. Any calibration procedures or adjustments necessary to assure accurate and reliable equipment operation will be performed at this time. d. P.M. procedures used to perform work under this contract are required to be submitted. Two copies of the P.M. procedures used must be presented in writing to the Contracting Officer within 10 days of contract issue. Failure to provide these procedures may result in delay of payment or non-payment. e. During the P.M. visit, the contractor shall replace such parts as may be necessary for the satisfactory functioning of the equipment. (See item #6 for quality of parts). f. The contractor shall call Biomedical Engineering Section by phone (843) 789-7396, at least two (2) days in advance to confirm that the equipment can be serviced at a time that is agreeable to the VA Contracting Officers Technical Representative (COTR) or his/her designee and the contractor. 3. Emergency Service: a. For each emergency service visit, initiated at the request of the COTR or his/her designee, the contractor shall furnish all labor, material, repair parts, (except those cited within Excluded Parts), equipment (not to include operating supplies), and travel necessary to correct equipment to operational condition. Contractor will be responsible for all required maintenance of VAMC Charleston sterilizing/washing equipment listed in purchase order. All work performed shall be accomplished in accordance with manufacturer's instruction including, but not limited to, adjustments, calibrations, cleaning, lubrication, testing, disassembly, check-out, replacement of worn or defective parts, etc. The contractor shall guarantee that at the conclusion of any emergency maintenance performed that the equipment will meet manufacturer's factory specifications. Contractors maintenance of the equipment must satisfy all requirements as set forth by NFPA and Joint Commission. b. After each service call, the contractor shall submit a service ticket containing each part installed to the VA Engineering Department office as noted above. c. The contractor is required to provide Engineering Department with any service representative's name, address, and phone number so the VA Medical Center can obtain emergency maintenance service after normal VA working hours. The contractor is responsible for notifying Engineering Department within 24 hours of any changes in the emergency response information. d. Emergency service coverage will be eight (8) hours a day, five (5) days a week, not including holidays. All service shall be performed during normal V.A. working hours of 8:00 a.m. to 4:30 p.m. Any service not covered under this contract must have prior approval of the Contracting Officer before any work is performed. Thirty (30) minute response time by phone and 24-hour response time on station is required. Response time is defined as time vendor is first notified until time vendor responds. For failure to respond to intervening service calls within twenty-four (24) hours after notification by the Contracting Officer's Technical Representative, a deduction of 1/10th of the quarterly rate will be taken for each day a machine is inoperable. Furthermore, failure to perform any of the services set forth in this contract will be considered for invoking provisions of Default. Contractor will provide 24/7 technical phone support The Contractor possesses proprietary data that require the use of the authority below. Steris Corporation is the sole source of this service and the only authorized distributor of their patented service(s)/product(s). It is believed that there is no comparable service on the market. NAICS Code: 339112 Surgical & Medical Instrument Manufacturing (Size Standard 500 Employees) This proposed procurement is for services for which the government intends to solicit and award with only one source in accordance with FAR 6.302-1(b)(1)(i), Only One Responsible Source and no Other Supplies or Services will Satisfy Agency Requirements. This notice of intent is not a request for competitive quotes. However, interested parties may identify their interest and capability to respond to this requirement no later than August 2, 2013, 4 p.m. EDT. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government.
