RFP SP330013Q5005 Attachment 2-Past Performance Questionnaire Attachment 1 - Schedule of Supplies or Services 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 - Streamlines Procedures for Evaluation and Solicitation for Commercial Items in conjunction with the Federal Acquisition Regulations (FAR) Subpart 13.5 Test Program for Certain Commercial Items. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Request for Quote (RFQ) and the RFQ number is SP330013Q5005 (See attached RFQ). 2. This notice incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-68 effective 26 June 2013 and Defense Federal Acquisition Regulation Supplement (DFARS) DPN 20130626 edition and Defense Logistics Acquisition Directive (DLAD) Current to Revision 5 and PROCLTR 2013-53. This Request for Quotation (RFQ) is being issued to establish a firm-fixed price requirement type contract for different sizes of Heat Sealable Bags (electrostatic and non-electrostatic discharge) for various CONUS and OCONUS DLA depots. 4. This solicitation requires vendors to submit pricing for each CLIN and each zone defined below. Delivery Orders will be shipped to destinations in Zones 1 and 2. The destination address will be provided in the individual Delivery Orders placed against this contract. F.O.B. Destination prices shall include delivery to all destinations within Zones 1 and 2, including delivery to the continental United States (CONUS) Consolidation and Containerization Points (CCP) within the respective zones if the ultimate destination is OCONUS or overseas. Destinations within the zones include, but are not restricted to CONUS Defense Logistics Agency (DLA) Distribution sites. Vendors may not propose alternate zones. The zones are defined as follows: Zone 1: Alabama, Arkansas, Connecticut, Delaware, Florida, Georgia, Illinois, Indiana, Iowa, Kentucky, Louisiana, Maine, Maryland, Massachusetts, Michigan, Minnesota, Mississippi, Missouri, New Hampshire, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Rhode Island, South Carolina, Tennessee, Vermont, Virginia, Washington, D. C., West Virginia, Wisconsin. Zone 2: Arizona, California, Colorado, Idaho, Kansas, Montana, Nebraska, Nevada, New Mexico, North Dakota, Oklahoma, Oregon, South Dakota, Texas, Utah, Washington, Wyoming. This RFQ is 100% small business set-aside. The North American Industry Classification System (NAICS) code for this project is 322220 and the size standard 500 employees. 6. PERIOD OF PERFORMANCE: The period of performance shall be on or about Base Year: 01 November 2013 through 31 October 2014 Option Year 1: 01 November 2014 through 31 October 2015 Option Year 2: 01 November 2015 through 31 October 2016 7. A technical package is required from all offerors and shall be provided with the quotes. Please pay special attention to the Instructions to offerors starting page 19. The instructions require various items of documentation to submit with your quote; such as technical capability information, past performance information, completed Representations and Certifications, etc. The evaluation criteria, starting page 23 explains how offeror's quote will be evaluated for acceptability/unacceptability. 8. ESTIMATED QUANTITIES : The quantities shown in the Schedule of Supplies/Services are estimated only. They represent the Government's best-known estimate for each line item. Payment will only be for the actual quantities provided to the Government at the price specified. If the Government requirements do not result in the quantities estimated; such event shall not constitute the basis for an equitable price adjustment under the contract. 9. VARIATION IN QUANTITY: VENDORS ARE CAUTIONED IN THE USE OF A QUANTITY VARIANCE. DLA'S CURRENT PAYMENT SYSTEM DOES NOT RECOGNIZE PARTIAL QUANTITIES ON A QUANTITY VARIANCE. THEREFORE, A QUANTITY VARIANCE NEEDS TO BE ROUNDED DOWN RATHER THAN UP IN ORDER TO BE PAID PROPERLY. FOR EXAMPLE, IF A VENDOR IS ALLOWED A PLUS OR MINUS 10% VARIANCE ON A QUANTITY OF 199, THE SYSTEM WILL ONLY ALLOW PAYMENT ON AN ADDITIONAL OR LESSER QUANTITY OF 19, NOT 20 (199 x.10 = 19.9). 10. SYSTEM FOR AWARD MANAGEMENT - A prospective awardee shall be registered in the System for Award Management (SAM) database prior to award, during performance and through final payment of any contract resulting from this solicitation. Offerors may obtain information on registration and annual confirmation requirements via the Internet at http://www.sam.gov. 12. REVERSE AUCTION : DLAD Provision 52.215-9023 (NOV 2011) Reverse Auction is included in this solicitation (see pages 26 and 27 of the RFQ for full text). 13. INVOICING AND PAYMENT - Invoicing and Payment will be made via WAWF. See DFAR Clause 252.232-7003 and 252.232-7006 found pages 12 and 13 of the RFQ. 14. ADDITIONAL WAWF INFORMATION FOR OCONUS SHIPMENTS - The contractor shall provide proof of delivery to the CCP Issuing Officer so that receipts may be posted in order to allow for payment through WAWF. 15. CONTRACT PRICING : CLINs 0001 through 0060 are for the Base Year, CLINs 1001 through 1060 are for Option year 1 and CLINs 2001 through 2060 are for Option year 2. 16. PACKING AND MARKING REQUIREMENTS : All heat sealable bags shall be packaged best commercial practice in accordance with ASTM 3951 and marked in accordance with MIL-STD-129P. All shipments must contain proper shipping documentation. Contractor shall attach, in a water resistant envelope, to the exterior shipping container a commercial packing list. All documents must include at a minimum, contract number and purchase request (PR) number for the item, stock number, quantity, and unit of issue. Omission of any data on the shipping documentation could cause a delay in receipt of goods and significantly delay payment. 17. MARK CONTRACT NUMBER ON ALL CORRESPONDENCE Contractor must mark the contract or purchase order number on all paperwork and shipments. The order number must appear on the exterior of the shipment. Failure to follow these instructions will hold up payment to you and could result in the return of merchandise at your expense. 18.
