- Attachment 6-WD 05-2017 Rev 17 eff 25JUN13 Attachment 5-C12 and Contract Flight Price Schedule Attachment 4-Required Inspections and Schedule Attachment 3- INSTRUCTIONS FOR COMPLETING AF FORM 1222 (7-12-2013) (2) Attachment 2-AF FORM 1222-AF EQUIVALENT (7-12-13) Attachment 1-PWS Dated 30 July 2013 This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals/quotes are being requested and a written solicitation will not be issued. The solicitation reference number is FA8051-13-RFQ-AlaskaBoilerInspections and is issued as a Request for Quote (RFQ). The 772 ESS/PKH Contracting Squadron, Tyndall AFB, Florida, requires the following: Boiler and UPV Inspection services at various locations in Alaska. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-67 effective 22 July 2013, Defense Federal Acquisition Circular Notice DPN 20130710 effective 10 Jul 2013, and Air Force Acquisition Circular AFAC 2013-0327 effective 27 March 2013. This acquisition is a 100% total small business set-aside under the North American Industry Classification System (NAICS) code 541330, Engineering Services. The business size standard is $14 Million. A firm fixed price contract will be awarded. The contract will be awarded with one Contract Line Item Number (CLIN). CLIN Description Qty Unit of Issue 0001 Boiler Inspection Services at 1 Job various locations in Alaska (See Attachment 4) Award will be made to the lowest price offeror that is determined to be technically acceptable. To be determined technically acceptable, the proposal must meet the requirements of the attached Performance Work Statement (PWS) (Attachment 1). The offeror shall submit a schedule for inspections. Please note within Attachment 4, there are certain inspections that must be done on required dates. There are inspections that have open dates (noted by the term "variable"), which the offeror must account for. The schedule must be submitted with the offer and take into account the required inspection dates and the variable inspections. The scheduled submitted with the offer will be binding. The inspections must begin on 3 September 2013 and finish by 1 October 2013. Additionally, to show the inspectors are qualified to perform the inspections in accordance with the PWS, the offeror shall submit the proper documentation to show the inspectors meet the requirements of the National Board of Boiler and Pressure Vessel Inspections and the National Board Inspection Code (section 2.2 of the PWS). Any offer deemed technically unacceptable, will be eliminated from competition. The estimated award date is 23 August 2013. The Government also reserves the right to make no award at all. Since award will be based on initial responses, offerors are highly encouraged to quote their most advantageous pricing in their initial response.
