- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12, Acquisition of Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation that will be issued. The solicitation number for this requirement is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1, Simplified Acquisition Procedures. QUOTERS ARE ADVISED THAT AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE QUOTES RECEIVED. Quoters should not assume they will be contacted or afforded the opportunity to qualify, discuss, or revise their proposals. Therefore, the initial quote should contain the best terms from a technical and price standpoint. All quotes must include the full name, address, and DUNS number of the business to be named on the purchase order as well as the full name, phone number, and email address of the person who can accept the final purchase order. THIS IS A TOTAL SMALL BUSINESS SET-ASIDE. The North American Classification System (NAICS) code for this procurement is 541990 -All Other Professional, Scientific and Technical Services- with a small business size standard of $14 Million. Quotes are due by Friday, August 9, 2013, by 3:00 PM EST and should be sent to the attention of H. Dale Bish via email to hbish@cdc.gov. Hard copies can be delivered via Fed Ex or UPS to H. Dale Bish, Centers for Disease Control and Prevention, Procurement and Grants Office, 626 Cochrans Mill Road, Pittsburgh, PA 15236. The Offeror must cite RFQ Number 2013-61708 in the email subject line and include it on the shipping label if sending hard copies. Evaluation and award of the purchase will be performed in accordance with applicable sections of FAR Part 12, Acquisition of Commercial Items, and FAR Part FAR 13, Simplified Acquisition Procedures. The Offeror must have an active registration with the System for Account Management (SAM), which replaced the Central Contractor Registration (CCR), prior to award of the purchase order in accordance with FAR Clause 52.204-7. The government intends to issue a Firm Fixed Price purchase order in accordance with FAR Part 13. Proposals must be submitted in two parts: (1) Technical Proposal and (2) Pricing Proposal. The Government will award a contract resulting form this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Technical and past performance, when combined, are significantly more important than price. EVALUATION CRITERIA The project includes the identification of existing work-related injury and fatality systems in the US forestry industry, the description of such systems, and an analysis of these systems' utility for planning, monitoring, and evaluating occupational injury and fatality prevention programs. The contractor will create a search and analysis strategy, conduct a comprehensive search of existing systems, analyze them, and submit a final report describing the methods used, the findings of the search and analysis, and conclusions and recommendations for how to best use or supplement existing systems for planning and evaluation of occupational injury and fatality prevention programs in forestry. 1. Demonstrated ability and experience accessing, interpreting, and analyzing secondary sources of scientific research data, such as scientific journal articles or academic reports. Experience with occupational safety or occupational health data is preferred. This could include a list of pertinent publications (e.g., journal articles in professional publications, books, book chapters), as well as presentations at scientific or industry conferences. 2. The successful bidder must have personnel with academic training and scientific research experience in epidemiology or professional experience with workforce data collection systems in the US forestry industry. 3. Demonstrated understanding of the objectives for this effort, underlying issues, scope of work, and the detailed requirements as evidenced by the completeness and soundness of the approach presented in the proposal. 4. Demonstrated capability in completing contracts of similar complexity for the government or private industry in a timely and successful manner. Questions must be submitted via email to H.
