Description

Synopsis Notice: The NCO7 (Network Contracting Office) is seeking potential sources capable of conducting reference laboratory testing services for patients of the Selma Outreach Clinic and Home Based Care (HBPC) based estimated quantities relevant to patient needs. This synopsis is for planning purposes only, and does not constitute a solicitation for bids/proposals. This is a single-award, indefinite delivery- indefinite quantity requirement. The NAICS Code is 621511- Medical Laboratories Description of Medical Services: The contractor shall provide laboratory testing services for patients of the Selma Outreach Clinic and Home Based Primary Care (HBPC) based upon estimated quantities relevant to patient needs. HBPC provides VA approved patient care from the patient's residence. Services are required for a base year with four 1-year option periods. Services shall be performed as non-personal services in which contractor personnel will not be considered VA employees. PLACE OF PERFORMANCE: Laboratory testing services shall be performed at the contractor's facility. Contractor laboratory testing facility must be within 10 mile radius of the VA Selma Outreach Clinic, Selma, Alabama. All interested parties shall submit a capabilities statement not to exceed (3) pages. Capabilities statements are due no later than August 7, 2013 at 4:30 pm (EST) and shall be submitted to the attention of Laurinda Millage via email: laurinda.millage@va.gov. Facsimile transmissions are not acceptable. The following information should be included: a.Name and address of the business b.Point of contact information to include name, title, phone number and email address c.Contractor's business size as well as designation as a small, disadvantage business (SDB), woman owned small business (WOSB), veteran owned small business (VOSB), services disabled veteran owned small business (SDVOSB) or HUBZone small business. The Government will review any and all capabilities statements submitted and determine if other qualified sources do exist that could perform the effort. Information received in response to this notice will be used as market research to determine the availability of commercial vendors capable of providing the requested services.
