- This is a SOURCES SOUGHT NOTICE - This synopsis is for information and planning purposes only and is not to be construed as a commitment by the Government. No contract will be awarded as a result of this Sources Sought announcement. The Department of Veterans Affairs (VA), Network Contracting Office (NCO 21) is soliciting information from qualified Service Disabled Veteran Small Business (SDVOSB), Veteran Owned Small Business (VOSB), or Small Business (SB) sources that possess the capability and experience required to provide occupational-related ergonomics evaluations for VA employees located at the San Francisco VA Medical Center, 4150 Clement Street, San Francisco, CA 94121, as well as other locations in downtown San Francisco, CA, Clearlake, CA, Eureka, CA, San Bruno, CA, and Ukiah, CA. This requirement includes performing ergonomic evaluations on employees from various departments within the VA. Evaluations shall be performed by a Board Certified Professional Ergonomist. All ergonomic evaluations and consultations shall adhere to the Ergonomic Guidebook framework as included in the Department of Veterans Affairs (VA) Center for Engineering and Occupational Safety and Health (CEOSH). A written report shall be submitted for each evaluation conducted. The anticipated period of performance for this requirement is for a base period of one year with four one-year options, total duration not to exceed sixty (60) Months. The North American Industry Classification System (NAICS) Code is 541690 and the small business size standard is $14 million. Respondents shall provide detailed information as to how they are qualified to perform this work including a description of the background, education, experience, and individual expertise of the Board Certified Professional Ergonomist who will be performing the evaluations. In addition, respondents are requested to submit the following information: (1) Name of Organization, (2) Address + Zip Code, (3) Telephone number, (4) Business status: SDVOSB, VOSB, or Small Business, (5) Point(s) of contact, (6) e-mail address, (7) DUNS number, (8) number of years experience in providing this service, (9) current and/or past performance references that identify the contract/order number, customer, contract value, period of performance, prime or subcontract, and a brief description of services provided under the contract/order number. Interested vendors shall notify the Contracting Officer of pending changes in small business status. The results of this announcement will be evaluated to determine if at least two SDVOSB, VOSB, or SB sources are capable of performing the proposed work. Respondents will not be notified of the results. Based on the responses received, the Contracting Officer reserves the right to determine the set-aside status. No reimbursement will be made for any cost associated with providing information in response to this solicitation. All responses must be submitted by e-mail to the point of contact identified below; no other method of transmittal will be accepted. Responses to this Sources Sought Notice shall be e-mailed to the Contracting Officer, William Tanksley, at william.tanksley2@va.gov. Responses must be received no later than 2:00 PM PST on August 13, 2013. Failure to submit all information requested will result in a contractor not being considered as an interested business.
