- REQUEST FOR QUOTATION Solicitation notice # D-3-D9-15-SE-R1A-000 The Internal Revenue Service has a requirement for various moving and reconfiguration of office cabinets, equipment, miscellaneous furniture, and wall items at the following location: Internal Revenue Service, 915 2 nd Avenue, Seattle, WA 98174. See attached Scope of Work for full details on required services. Primary Point of Contact on site is Debbie Maund: 206-220-4742. Due to IRS Furloughs the site will be closed on August 30 th, so services will not begin on this date as stated in Scope of Work. Service is tentatively scheduled to now begin on the next weekend of September 6 th but is subject to change. Following award, the Contractor shall call Debbie Maund prior to beginning service to finalize the full schedule. This solicitation is for Commercial services and is issued as a Request for Quote. The following additional provisions are applicable to this solicitation and included by reference: 52.212-1 Instructions to Offeror-Commercial 52.212-2 Evaluation-Commercial Items (a) Award will be made to low price, technically acceptable offeror 52.212-3 Offeror Representations and Certifications-Commercial Items The following clauses are applicable to this solicitation and will be incorporated in to the awarded contract: 52.212-4 Contract Terms and Conditions-Commercial Items 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items Internet Payment Platform Clauses - See attached The current applicable wage determination at the time of this solicitation shall apply to this awarded contract in accordance with the FAR 52.222-41 Service Contract Act of 1965. If you are interested in submitting a quote, please review the enclosed documents. All submittals must be completed and forwarded to the Contracting Officer, Joseph N. Laughlin, for consideration of your offer. You may submit your quote via email at Joseph.N.Laughlin@irs.gov as soon as possible but no later than 27 August 2013 by 04:30 PM PST. Please respond to the following provision and provide response with your quote. Failure to include response may result in your quote being deemed unresponsive and ineligible for award: Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction under Federal Law (FEB 2012) (1) The offeror does [ ] does not [ ] have any unpaid Federal tax liability that has been assessed, for which all judicial and administrative remedies have been exhausted or have lapsed, and that is not being paid in a timely manner pursuant to an agreement with the authority responsible for collecting the tax liability. (2) The offeror, its officers or agents acting on its behalf have [ ] have not [ ] been convicted of a felony criminal violation under any Federal law within the preceding 24 months. If you have any questions, pertaining to the Statement of Work or other attachments, they should be submitted in writing via e-mail to Joseph.N.Laughlin@irs.gov. Questions should be received with sufficient time to respond prior to proposal due date. There will be an organized site visit of the site on 21 August 2013 at 09:00 am. Please contact Debbie Maund, Deborah.J.Maund@irs.gov, 206-220-4742, to arrange access to the site at the time of the visit. While the site visit is not mandatory, it is highly encouraged as the offeror is responsible to verify all site conditions. The Government intends to award a firm fixed price contract. Your quote should be submitted as a single line item amount to include all costs to provide labor and services in accordance with the attached Statement of Work. Rates may be included for costs based on estimated weight and may be invoiced at actual weight but the estimated weight should be included in offer for evaluation purposes. Contractor shall provide all labor, materials, equipment, transportation, supervision, and all other necessary items for a complete and useable product in full compliance with all plans, drawings, and specifications as defined in the solicitation documents.
