- Question Responses for OPM15-13-R-0004 Reading Room Nondisclosure Agreement Amendment 0002 The OPM Responses to Credit Search Services received by the closing date of July 3, 2013, are hereby posted below for your information and appropriate action. The questions and responses have been arranged following the order of the Request for Proposal. Several of the reference documents listed in Section J of the RFP are available for review ONLY in the Reading Room located at OPM's facility at 1137 Branchton Road, Boyers, Pennsylvania 16018. No new questions may be submitted. However, follow-up questions may be considered as described in the attached responses. However, the determination of the contracting officer as to whether a question is "follow-up" or is actually a new question will be final. No follow-up questions will be accepted after 3:00pm Eastern Time on Friday, August 9, 2013. Access to the Reading Room will be provided upon written request. Submit requests via email or fax not less than two business days in advance of the planned visit to the Contract Specialist named in this notice. Requests ONLY include the following information for each visitor: full name (first, middle & surname); social security number; date of birth; place of birth; organization or company represented; planned date of visit and approximate time of arrival. Visitors will be required to complete and submit a copy of the Reading Room Nondisclosure Agreement form, which is an attachment to this amendment. Proposal submissions must comply with the requirements described in Section L. Hard copy proposal submissions must be delivered to: Boyers Contracting Group Attention: Walter Werner, Contract Specialist 1137 Branchton Road Boyers, PA 16018 The closing date and time for submission of proposals will be 3:00PM EDT on Thursday, August 29, 2013.
