Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Pacific Regional Contracting Office, Tripler Army Medical Center (TAMC), Honolulu, HI 96859, intends to solicit and award a contract for Catholic Clergy Services. This is a request for quote (RFQ). This solicitation number is W81K02-13-T-0137. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. This acquisition is a total small business set-aside. The NAICS code is 813110, and small business size standard is $7M. This RFQ contains four (4) CLINs. 0001 - PATIENT DAILY PASTORAL CARE & MASS 0002 - ON-CALL SVCS, NON-DUTY HOURS 0003 - CLERGY WEEKEND SERVICES 0004 - CMR SEE ATTACHMENT 1 for additional CLIN information The Government intends to award a Firm, Fixed-Price contract for a period of performance of 01 October 2013 through 30 September 2014. Contract Requirements - The successful offeror will be required to perform the services specified in the Performance Work Statement (PWS) included as an attachment. - 52.212-1, Instructions to Offerors -- Commercial Items, and its addendum apply to this acquisition - 52.212-2, Evaluation -- Commercial Items and its addendum apply. The following factors will be evaluated; Factor 1. Technical Expertise, Factor 2.
