Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is LAW-13-Q-0015 for Digital Radiographic Equipment Services for the Carestream DRX-Evolution located at the Anadarko Indian Health Clinic, Anadarko, Oklahoma. This solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. This acquisition is full and open. The NAIC code is 811219 and the small business size standard is $19M. The FOB point is Anadarko Indian Health Center, 115 N.E. Oldtown Drive, Anadarko, Oklahoma 73005. Evaluation will be based on price and past performance. Offerors are to include a completed copy of the provision found at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. Contractor shall provide: A. Customer may report issues or obtain technical support 24 hours a day, 7 days a week. Contractor shall respond to Customer by phone within 30 minutes of the start of the Coverage Period. B. Contractor will use remote management service ("RMS") allowing contractor to remotely monitor, diagnose and resolve a variety of Equipment and Software issues, and remotely install Updates, through a secure, encrypted internet connection or a secure remote service access ("SRSA") virtual private network ("VPN") connection. If Contractor is not able to resolve an Equipment issue remotely, Contractor shall dispatch trained and supported field engineers to diagnose, repair and calibrate the Equipment on site during the Coverage Period for Equipment covered under the service plan. C. Genuine Manufacturer replacement parts are included for all Equipment repairs and maintenance covered by service plan, including tubes and detectors, unless otherwise specified. Contractor may use reconditioned or used parts to meet its service obligations, however, Contractor warrants all such reconditioned or used parts under the same warranties applicable to new parts. Upon completion of a service call, Contractor's field engineer shall certify that the Equipment meets specifications. D. The targeted time for a Contractor field engineer to arrive on site to initiate Equipment repairs is three hours from commencement of the Coverage Period. E. Contractor shall not charge Customer additional hours for repair calls that commence during the Coverage Period and continue beyond the Coverage Period. F. If Contractor determines that the Equipment or Software requires a modification to address patient or operator safety, Contractor shall install such safety modifications at no additional charge. H. Contractor shall install updates, and if applicable Releases or Upgrades, during Normal business Hours. I. If the Manufacturer's basic software refresh for an item is purchased, and Manufacturer issues a Release or Upgrade for that item during the Service Period, Contractor shall provide that Release or Upgrade to Customer at no additional license fee. J. Equipment covered by First Call or Full Partnership service plan, Contractor shall provide the biomedical department with technical training. Contractor shall also provide telephone support to the biomedical department for Equipment repairs. Contractor shall provide on-site service, if required. K. Contractor will provide Customer with IP address changes, native DICOM connections, technical training and other non-warranty support and services upon request. Contractor will provide IP address changes and native DICOM connections at no additional charge. L. Service will include an Accident Protection Plan for the DRX's (5). Secure Remote Service Access. A VPN connection is a requirement for remote service and support and it is the preferred connection method. If needed, Contractor shall provide Customer with a VPN device and configuration to provide a secure encrypted connection from Contractor's support network to Customer's Equipment.
